SUMMARY:
While working the Electric Forest Festival, I was dispatched to the listed venue in reference to an
overdose/possible DOA.
VENUE:
06/27/2016 AT 0938
COMPLAINANT:
ARRIVAL ON SCENE:
I arrived on scene and was able to see EMS personnel loading the victim onto the stretcher. CPR
was in progress. EMS personnel stated the victim had no pulse and was not breathing. Sgt. Harvitt of
the Rockford Post was already on scene assisting. let which I then
secured in my patrol vehicle.
Oceana EMS 831 responded to the scene. The individuals working were Trevor Lasater and Scott
Domin.
D/SGT. RIOS:
D/Sgt. Rios arrived on scene and began his investigation. While D/Sgt. Rios conducted his
investigation, Sgt. Harvitt and I were able to get consent to search the scene, and any belongings on
scene.
SEARCH OF SCENE:
I conducted a search of the entire scene. While searching the scene, I located several drugs and
several drug paraphernalia items. These items were all seized and added into property. After I
concluded my search, I stood by for scene security while D/Sgt. Rios finished his investigation.
PHOTOGRAPHS:
Photographs of the scene were taken by Sgt. Harvitt. The photos were later uploaded to the MSP
Repository.
FIELD TEST:
The drugs were all brought back to the Newaygo Detachment where they were field tested. The tests
were conducted using the Nark II test kits and utilizing the instructions on the back of the box. All
tested items yielded positive results.

On June 27, 2016, ran into at about 6 a.m. At the time, seemed
okay. kept asking her to help him find some ketamine, but indicated she's not into
that stuff. would text her phone and ask her if she found anything, and she kept telling him, no,
because she did not try.
I asked if she knew if he was doing any other drugs, other than ketamine.
said he was doing cocaine, MDMA, and she was pretty sure he dropped a hit of acid; however, she's
not positive. I asked if she knew where he got these things from, which she indicated she
had no clue, further indicating people would walk up and down the aisles and just sell it.
went on to say some nice ladies brought around some donuts Monday morning.
doesn't like donuts, so they were going to wake and give him a donut, and they
found him face down in that "blue thing." (The blue thing was a blow up chair, which was seized.)
thought this all occurred about 8 a.m.
For clarification, I asked where she last saw She stated she saw him about 6 a.m.
as they were walking out of the venue. went on to say she saw him a couple times and
he seemed okay and every time she saw him, he would ask her if she found any ketamine.
I asked when she saw take cocaine. She stated she saw him snort it a couple
times over the weekend. I asked when she saw take the molly. stated
she didn't think she saw him last night; however, it was Saturday night. I asked about the

indicated he, some girl they met, and had taken some coke around 4 a.m. on June 27,
2016, as well as some ketamine. also recalled around 3 a.m. a white male, approximately 25
years old, was overheard selling stuff. could not provide a further description and indicated it
was dark out. indicated walked off and seemed okay; it was still dark at the time.
went on to say that said was sleeping behind his car. They tried to wake him,
and he was facedown. They turned him over and was blue.

indicated and he drove up together. They didn't know before this weekend.
They know that he slept yesterday at the tent, as well as this morning. About 6 a.m. he saw
near the porta-john and seemed to be okay. They went to wake him up at 8:30 in the morning,
as he slept behind their vehicle.
indicated they saw him take molly every day and talked about ketamine and coke;
however, he didn't think he saw him take those.
When was found in the morning, described as lying on his left side, and was cold
to the touch when he checked the pulse.

said he woke up this morning to clean up the campsite and told to wake up
acknowledged over the weekend he did cocaine, ketamine, and molly.

indicated she did not know before this weekend; however, they were going
to give him a ride back.
indicated was asking for some ketamine. They last saw him around 4 a.m., when it
was still dark. At that time seemed to be okay. was asking everyone if they had any
ketamine as they walked by.
I made telephone contact with DR. OCHES who advised the medical examiner investigator was
JANICE SCHILLER. I contacted JANICE SCHILLER on June 27, 2016, who advised
was pronounced dead at 10:16 a.m. by the ER doctor; however, she did not have the
E.R. doctor's name.
I asked SCHILLER who transported to Blodgett for the autopsy. SCHILLER indicated
the Harris Funeral Home, transported to Blodgett.
I advised SCHILLER the witnesses indicated was found face down and/or on his left
side. Further indicating the witnesses either observed or believed was taking
ketamine, cocaine, molly, and possibly acid. I asked if she would pass that on to the medical
examiner who was doing the autopsy. I asked SCHILLER when the autopsy was going to be
performed. SCHILLER indicated the autopsy was already started.
The medical examiner investigator did advise she was told there was an effort to resuscitate at the
hospital.

I contacted Oceana EMS on June 28, 2016, to find out who was operating Rig 832, and transported
from the scene to the hospital. I was advised by LANCE COREY that
TREVOR LAFATER and SCOTT DOMAN transported him to the hospital. COREY advised a court
order or subpoena would be needed for a copy of their report.
I contacted Lakeshore Emergency Hospital on June 28, 2016, to identify who pronounced
dead. I was advised DR. CARL MATALF pronounced dead.
LT. KANITZ advised father was notified of the death; complaint #72-2168-16.

I provided JULIE FRAISER a copy of the affidavit and search warrant and she provided me with a
copy of the Oceana EMS report. FRAISER was given a return and tabulation.
I made contact with KATHY BOONE from the records department and gave her a copy of the search
warrant and affidavit. BROONE indicated she would call when the records were ready.

A search warrant was drafted and a search was conducted by the Michigan State Police CCU
complaint # CCU-1877-16.
The results of the search was placed on a disc and entered into evidence as item 21.
The below is a copy of a few of the messages found on the phone which appears to be both
purchasing and selling drugs.
