Defining Midwifery

Midwifery
According to International Confederation of Midwives
(ICM), “midwifery is the profession for midwives, like other
profession midwifery has a unique body of knowledge, skills
and professional attitudes that were drawn from other health
professions, but practiced by within a professional
framework of autonomy, partnership, ethics and
accountability” (ICM, 2017 p.1).
Moreover, the ICM also describe midwifery as an approach
to care of women and the newborn focused on:
a. optimizing the normal and biological, psychological,
social and cultural process of childbirth and early life
of newborn;
b. Work in partnership with women, respecting the
individual circumstances and views of each women;
c. promoting women’s personal capabilities to care for
themselves and their families;
d. Collaborating with other midwives and other health
professionals as necessary to provide holistic to meet
woman’s individual needs.
Midwife
A midwife is a person or an individual who is admitted
in a duly accredited and recognized institution who offer
midwifery program and has been successfully completed all
the requirements specified in the course and passed the

licensure examination to become a registered or legally


licensed to practice midwifery.

In the Philippines, the Commission on Higher


Education (CHED) is responsible in regulating
midwifery education while the Board of Midwifery,
under the Professional Regulation Commission,
regulates the practice of the profession by administering
licensure examination to qualified students.

Brief history of midwifery practice

Midwifery can be considered as one of the oldest


profession. The study of Brawi, Richter & Habib (2013)
reveals that practice of midwifery existed as early as the
Stone Ages and continued to be practiced across different
Eras starting from the Biblical Era (2200 BC – 1700 BC)
until the Modern Era (12th – 16th Century). During the modern
period, the midwifery profession started to be regulated and
formalized. The renowned figure of nursing profession,
Florence Nightingale, is thought to teach midwifery during
her time.

In the Philippines, in the early 1900’s midwifery is


being practiced by what we “hilots”, these hilots are
community residents who do not have formal education to
midwifery and learned the art of assisting childbirth
through apprenticeship. However, this practice has been
criticized because of the risk of complications. In 1922,
Dr. Jose Fabella, the Secretary of DOH at that time, started
to formalize midwifery education by founding the first
midwifery school in the Philippines, founded in Sampaloc
Manila. After the founding of the first midwifery school,
another school was founded in Cebu, Bacolod and Negros
Occidental.

The first midwifery school founded in 1922 is still


offering midwifery program to this day.

Objectives of Dr. Jose Fabella in founding the Midwifery


School

1. To train young woman in midwifery to gradually replaced


the unlicensed midwives or hilot.
2. To train doctors and rural assignments
3. To give health service and education with emphasis on
maternal and child health

Historical development of Midwifery Program


The midwifery program is a one (1) year term or twelve
(12) months from 1922 to 1951. The realization of the
important roles of midwives in the healthcare system, the
one (1) year term became a one (1) year and a half (1 year
and 6 months). In 1974, the Chairman of the Board of

Midwifery (BOM) in person of Dr. Ricardo Gonzales, further


increase the duration of the midwifery program and it became
a Two (2) Year Midwifery Program. The Two (2) year diploma
in Midwifery Program became the standard in the midwifery
education until the Memorandum Order (CMO 33, s.2007) issued
by the Commission on Higher Education (CHED) that the
midwifery program should become a four year or a bachelor
degree program.

Midwifery Regulation
From the founding of midwifery education, midwifery is
regulated under the umbrella of medical education. When the
law RA 2644 was enacted in 1960, the Board of Midwifery
(BOM), under the Professional Regulation Commission (PRC),
was created and the office of the BOM became the regulatory
body for midwifery profession in the country including the
conduct of board examination for midwifery graduates.

Man: its characteristics, needs and


rights.

Concept of Man

Concept – a generic idea generalized from a particular


instance or subject.

Sister Callista Roy

Sister Callista Roy is an American nursing


theorists that conceptualizes Man as a
Biopsychosocial and Spiritual being who is in
constant contact with the environment.

In her concept, she pointed four (4)


characteristics of Man which are Man as a
biological being, as a psychological being,
social being and a spiritual being.

1. Man as a biological being is exemplified that "man is


like other man", all human being has similar features
such as having physical bodies and needs, although each
might differ in physical features (such as skin color,
height, etc.) and preference in needs.

2. Man as a Psychological being, this concept is a complex


part of every human being that can hardly quantified.
Psychological aspect can be broadly characterized by
man’s ability to think, feeling and expressing emotions.
Combination of these made a concept that man as a unique
being.

3. Man also is a Social being, this concept means that human


being constantly interacts with other beings as well as
to cooperate with each other in order to achieve a common
goal.

5. The last characteristic is man as an spiritual being,
most human being, if not all, believe worship supreme
being that serve as a guiding light and influences the
way they live their life, though this supreme being may
be differ depending on the belief of each individual.
However, as spiritual being doesn’t only pertain to those
who believed in a supreme, even those who do not belong
to specific religion or worship a certain being can become
spiritual.

Another concept of Man that is proposed by Sr. Roy stating


that man as an Open System who is in constant interaction with
a changing environment. In conjunction to the aforementioned
characteristic of Man (Biopsychosocial being), man itself and
the environment creates an interactive system in which
produces a cycle of input and output process. Both the
individual and the environment serves as a stimulus that serve
as the input and direct the response of the individual, output,
as its adaptive response.

For an example: high environmental temperature


(input/stimuli) stimulates the body to perspire and make
individual to feel thirsty, as a response to this, the
individual tend to go to a cooler environment or drink more
water to stay hydrated and cool (output).

Martha Rogers

Martha Rogers is also an American


theorists that developed the “Science of
Unitary Being”. Based on the theory of
Unitary Being, Martha Rogers proposed the
following concept:

1. Man is unified whole composed of parts which are


interdependent and interrelated with each other.
This concept states that man functions as a whole
despite that the body is composed of different parts.
Despite of these individual parts, these parts are
interdependent and work with each other in order for the
body to function. For an example, in flexing an arm, in
order for the arm to flex in needs an interdependence and
coordination of different body parts such as the brain
(sends signals to the muscles located at the arm), muscles
and the bones.

2. Man is composed of parts which are greater than and


different from the sum of all his parts.
This concept exemplifies that human beings are
greater than the whole body combined. Man’s ability to
think, to create and innovate, and to express emotions
are only a few of the man’s ability that cannot be
measured using the combined parts of human body.

3. Man composed of Subsystem and Suprasystem.

These two systems described the internal and external


environment of Man.

The subsystem, is a composition of the whole body, it


begins with the cell that serves as the smallest functioning
unit of the human body, a group cells will make the tissues,
group of tissues will make the organ, group organs will make
the system, and group of systems (respiratory system,
cardiovascular system, integumentary system, etc.) make the
whole human body.

The suprasystem on the other hand represents the system


that is present outside the human body. The suprasystem begins
with an individual as the smallest unit, group of individuals
will make the family and group of families make the community.

Florence Nightingale

Florence Nightingale is known as the


“Lady with the Lamp”, she is a British nurse
and known to modernize nursing. Her theory
revolves around the idea that human
environment has a great influence to human
health. She also conceptualizes man as an
individual with a reparative processes to deal with disease
and desirous for health but passive in terms of influencing
the environment.

Virgina Henderson

Henderson is also an American nurse


theorists that conceptualizes man as a whole,
complete and independent being who has 14
fundamental needs and the role of healthcare
providers is to assist individuals to meet
these fundamental needs. Furthermore,
according to Henderson, man (person) is a
complete and independent being with a bio-sociological and
spiritual component, a similar concept with Sr. Callista Roy.

The characteristics or attributes of Man

1. The capacity to think or conceptualize on the abstract


level – according to this attribute, supports the concept
of Martha Rogers stating that “Man is composed of parts

Human Needs

Human Needs
Human needs is referred as the driver of people’s action,
the motives behind human behavior.

This needs can be physiological or psychological


requirements for the well-being on an individual or necessity
in order to live. Needs can be ranked on an ascending scale
depending on the essentiality, of the needs to the survival
of human beings.

Needs can satisfied either in healthy or unhealthy ways.


Healthy ways in meeting the needs are those that are not
considered to be harmful to self and others when in it conforms
to the individual’s sociocultural values or is not within the
law. Individuals who appropriately satisfies their needs are
tend to be healthier, happier and more effective.

Healthy ways of satisfying needs, such as food may include


eating of vegetables, unprocessed foods, and organic foods.
Unhealthy ways of satisfying needs such as food may include,
eating of fast foods, fatty foods, canned goods, etc.

Other needs can also be met either health or unhealthy


ways.

The basic human needs

1. Each individuals has unique characteristics, but certain


needs are common to all people
2. A need is something that is desirable, useful or necessary.
3. Human needs are physiologic conditions that an individual
must meet to achieve a state of health or well-being.

Characteristics of Basic Human Needs


1. Needs are met relative to the priorities of the
individual.
Example: A mother can give up her own personal needs
in order to satisfy the need of her child.

2. Needs may be met in different ways.


Example: a patient who is unable to swallow can
satisfies the need for food through feeding using
nasogastric tube.

3. Needs is stimulated by external or internal factors.


Example: Thirst can be stimulated by insufficient amount
of body fluids (internal) or as a result of a cot
environment.

4. Needs maybe deferred.


Example: The need to eat can postponed when an
individual is watching his favorite TV Show.

5. Needs are interrelated. Needs cannot be met unless related


needs are also met.
Example: The need for hydration is influenced by
decrease in the body fluids brought about by urination
and perspiration.

6. Failure to meet needs may result to occurrence of illness.


Example, not drinking sufficient amount of water may
lead to dehydration.

Maslow's Hierarchy of Needs


Abraham Maslow is known to be one of the most renowned
psychologist. He was known for creating the Maslow’s Hierarchy
of Needs. Maslow’s Hierarchy of Needs are arranged into a five
levels resembling to a pyramid.

The needs located in the lower level of the


pyramid are considered as the most essential or
important compared to other.
The needs located at the lower level of the
hierarchy must be satisfied first before going into
the higher level or next level.

Starting from the bottom of the pyramid, the five levels


of needs according to Maslow’s are:
1. Physiological Needs,
2. Safety Needs,
3. Belonging and Love Needs,
4. Esteem Needs, and
5. Self-actualization needs.

Self-actualization is considered as the highest


level of based on the hierarchy and satisfying this

that a self-actualization is the essence of mental
health.

Criteria of mentally health person according to Marie Jahoda.


1. The attitudes of a person towards himself is positive,
self-reliant, self-confident and self-accepting
2. The person can become aware of the meanings of his action
through introspection.
3. A person's self-concept is similar to that which others
have of the person
4. The person can accept himself
5. The person has sense of identity, that is know who he is
and the same time has few doubts about it.
6. The person changes and grow throughout life.
7. The person acts as a unified manner, that is, his behavior
is constant throughout.

According to William Glasser there are two needs that are


fulfilled by a mentally healthy person.
1. The need to loved and be loved.
2. The need to feel that one is worthwhile to self and

Rights
Rights are something that is owed to an individual
according to just claims, legal guaranties, moral and ethical
principles.

Three types of Rights

1. Welfare Rights (also called Legal Rights)


This are the rights that are guaranteed by Law.

Sources of Law
a. Constitutional Law (The 1987 Philippine Constitution) –
this is considered as the supreme law where all the laws
that are enacted is based.
b. Statutes (Statutory Law) – this are laws that are enacted
by the legislative body (Congress and Senate, it also
include charters and legislation in the municipal level,
administrative rules and orders, presidential issuances
or executive orders, etc.)
c. Treaties and conventions
d. Judicial Decisions – only the decision of the Supreme
Court (the highest court) establish jurisprudence and are
binding to all other lower courts.

2. Ethical Rights (also called moral rights)


This rights are based on moral or ethical principles and
usually do not require power of law to be enforced. This are
moral principles that govern the behavior of individuals as
well as conducting activities.

This are rights that are based on fundamental belief in

rights.

Human Rights
Are rights which as a person's due as a member of society.
They are not earned and exist independently of law or public
