NOTICE INVITING TENDER

for supply of
FIRST AID KIT FOR OUR BANK BRANCHES/OFFICES

Dear Sir (s),

We invite separate sealed competitive tender/quotation in two parts i.e. Technical

bid and financial bid from firms/suppliers/manufacturers for supply of First Aid Kit to our
Branches/Offices numbering about 156 (155 Branches and Zonal Office).

INSTRUCTIONS - TERMS & CONDITIONS:

1) Sealed tenders/quotation should be addressed to the Zonal Manager, Bank of India,

Andhra Pradesh Zonal Office.
2) Copies of the tender/quotation form can be obtained from Bank of India, Andhra
Pradesh Zonal Office, 28-2-48, 1st floor, Dasapalla Complex, Suryabagh,
Visakhapatnam-530020. The interested firms may also download prescribed forms
from our website www.bankofindia.com
3) The rate shall be filled in the bill of quantities both in words and figures.
4) No tender/quotation will be accepted after the aforesaid date and time under any
circumstances whatsoever.
5) The sample of the First Aid box and contents should be submitted along with the
quotation to make sure it is of the required type.
6) Sample of the First Aid Box which are not as per the specification mentioned will not be
accepted.
7) The tender/quotation will be rejected if not sent in sealed covers
8) Two separate sealed covers for financial and technical bid super scribed by tender for
‘‘First Aid Kit for branches/offices’’ should be sent and in no case, a single sealed
cover for both will be entertained.
 9) The last date for submission of tenders/quotation duly filled in sealed cover is
19.05.2015 latest by 17.00 hrs.
10) The tenders received in prescribed time limits will be opened on 20.05.2015 at 15.30
hrs in the Zonal Office in the presence of concerned bank authorities and authorized
representatives of the tenderer who can take decision on the spot in the matter. The
time and date of opening of the tender may be changed at discretion of bank authorities
and will be intimated to tenderers at above said office.
11) Date/time of opening of Part B of the tender containing price bill of quantities shall be
advised to the technically qualified bidders individually.
12) The validity of the quotes mentioned in the price bid shall be for a minimum period of 30
days.
13) On ascertaining the L1 bidder, the bank shall issue order and enter into a separate
contract for execution of the as by way of a separate agreement which shall be signed
by the successful bidder. In case of the L1 bidder refusing to execute the contract, the
bank shall at its option award the contract to the L2 bidder, in which case the L2 bidder
shall agree to execute the contract at the rates quoted by the L1 bidder. Non-
agreement of the L2 will give an option to the bank to re call the tender and come out
with a fresh proposal/ otherwise as to be decided by the bank.
14) The bank reserves the right to accept or reject in part or full any or all offers
without assigning any reason whatsoever.

PART A – TECHNICAL BID

PART B - FINANCIAL BID

The financial bid quoted should be inclusive of all taxes, duties etc and shall consist
of the following:

1. Per unit price (to be quoted as per Annexure 2)

- The Firm should quote the rates for all the items.

2. Annexure 2 containing the list of items/medicines should be submitted along with

Part B (financial bid)

3. Courier/Delivery charges
- Delivery charges for delivery of the first aid kit to the respective branches should
also be quoted.
