MINUTES OF IMPLEMENTATION COMMITTEE MEETING #41

DATE : 20th July 2016

TIME : 1130 – 1230 hours

VENUE : East Wing Boardroom, Level 2

1.0 Introduction

The purpose of this meeting is to discuss on the updates of safety and health issues,
documentations, and other related arising issues.

4.0 Opening and welcome

The Secretary (JH) welcomed all the Committee Members and called the meeting to
order at approximately 1135 hours.

5.0 Matter arising from the minutes

The previous minutes were discussed and confirmed with no amendments.

7.1 Incident / Accident Reporting

JH briefed the committee on the summary of the incident statistic that involved
team members and in house contractors for April 2016 to June 2016.

The followings are the details:

7.2 Incident case 1: Incident Investigation Report 05-16-002(O)

JH highlighted previous incident that happened approximately 0745 hours on


22.05.2016 that Susie Teng from Event Coordination Department slipped and
fell while she was walking towards East Wing Lift Lobby (LP 15 & 16), Level P1

from Traders Seasonal Parking Lot. She walked through the slippery area
where at the same time, a Housekeeping staffs was attending the cleaning
work. The accident has caused pain on her left foot and thigh.

i. Immediate cause:
The accident had caused pain on her left foot and thigh.

ii. Root cause:


- Oil spillage from the car after left parking
- Staff did not bring enough cleaning signage to be placed on site.
iii. NCR Status: SYSF005-NCR-05-16-013 - Closed

iv. Corrective Measure:


- Place 6 Caution signage at Security check points in case any wet
floor or oil spillage.
- Brief to all team members on the cleaning the oil spillage.

7.3 Incident case 2: Incident Investigation Report 06-16-001(O)

JH briefed that at 1930hours on 07.06.2016, it was reported by Azrul Azhar Bin


Ahmad Zubit, Asst. AV Manager that Azhar Bin Zaini, AV Supervisor had injured
his right toe. The incident happened while he was arranging others motorcycle
at Motorcycle Parking Area, P1 Level, East Wing when his foot accidently hit the
motorcycle stand. This had caused his right toe nail torn off.

8.0 External Case (April 2016 – June 2016)

8.1 Incident case 3: Incident Investigation Report 04-16-001(OE)

JH briefed that at approximately 1802 hours on 11.04.2016, it was reported by


Muhammad Zulkifli B. Che Kassim that he was hit by a shackle on his shoulder
that fell from the Plenary Hall Catwalk. During the incident, he was pushing a
cable box on the Plenary Hall Stage. He claimed that he was not injured.

i. Immediate cause:
Near miss - falling object.

ii. Root cause:


No proper clean-up of items after set up and team member in-charge for
the set up overlooked on the leftover shackle.

iii. NCR Status: SYSF005-NCR-04-16-012 - Closed

iv. Corrective Measure:


a) Rigging accessories to be put into flight case during setup/dismantle.
b) Plenary Hall catwalk area inspection will be done before and after
event.
c) Training and awareness for promotion of safe working environment.

8.2 Incident case 4: Incident Investigation Report 05-16-001(OE)

JH briefed that at 0930 hours on 15.05.2016, it was reported by Azril Bin Ayub
that he accidently cut his left ring finger while he was cutting carpet with a
penknife during the build-up day 1 of 2 for the event of SMIDEX 2016 at the
Booth K01-04, Exhibition Hall 4, Ground Floor, West Wing.

i. Immediate cause:
Minor cut on his left ring finger.

ii. Root cause:


Untrained personnel & improper method use of knife.

iii. NCR Status: SYSF005-NCR-05-16-001 - Closed

iv. Corrective Measure:


a) Training on the Work Instruction Procedure – PPE
b) Safety awareness briefing for each event – Main contractor & sub-
contractor.

8.3 Incident case 4: Incident Investigation Report 05-16-003(OE)

JH briefed that at 1220 hours on 27.05.2016, it was reported by Tricia Bok Rhu
Yen, PICO International Sdn Bhd and Chai Yi Lin, Brandme Associate Sdn Bhd
that backdrop structure of Booth No. #G12 (Prudential, build by PICO) and
Booth No. #G13 (Eco-World, build by Brandme) had collapsed at Exhibition hall
5, Ground Floor, West Wing. The incident happened when one of the Brandme
staff requested to remove temporary support for Prudential backdrop that
interrupted their works. After PICO staff removing the support, Brandme was
trying to combine their backdrop to the Prudential backdrop without realizing the
temporary support has not been installed by PICO. It was further informed that
one of the PICO staff was hit by the backdrop, however no injury reported

i. Immediate cause:
Booth structure unbalanced and collapsed.

ii. Root cause:


Miscommunication between contractors & no temporary support
installed.

iii. NCR Status: SYSF005-NCR-05-16-018 – Closed

iv. Corrective Measure:


a) Support was added on-site for both walls.
b) To reinforce future preventive action on minimum thickness in the
back wall for all contractors.

8.4 Commuting Incident (April 2016 – June 2016)

8.4.1 Case 1
JH briefed at approximately 1435 hours on 16.04.2016, it was reported that Noor
Hasanah Bt. Abdul Hakim had involved in a road accident at Jalan Ampang. The
incident happened while she was on the way to clinic from work place when a bus
cutting out from the junction and caused her to fell from her motorcycle. Consequently,
the incident has caused fractured on her left leg.

8.4.2 Case 2
JH briefed at approximately 07:40 hours on 08.06.2016, Ahmad Fadzeli B. Jabaruddin
from SHE Department had involved with a road accident on his motorcycle at Jalan
Ampang, Kuala Lumpur. The incident happened while he was on his way from his home
to the workplace when he hit side of a car that suddenly changed lane.
Consequently, the incident has caused him sprain on his left knee.

11.1 JH briefed the updates workplace inspection finding for 2 nd Quarter Y2016
to all SHC.

11.2 JH briefed that few group Safety Committee members did not complete
their workplace inspection in Quarter 2.

12.0 ERT Training program

12.1 JH highlighted Quarterly ERT training as follow:
