Greetings!

We, the students of BSN 3B-D of the University of the Cordilleras are having their community health
nursing (CHN) II this first semester of year 2021-2022. Despite the concept delivered via online platform,
the related community duty component requires the students to conduct community immersion but since
it’s a pandemic we have decided to gather data confined in the records of barangay for the Community
Diagnosis as partial requirement of our course. This will help us achieve our goal to help and guide
residents of the barangay towards a health secure future. We plan to collect and analyze data to determine
issues in the community that need to be addressed, increase awareness on community issues, especially
regarding health, respond to health problems of individuals in their complexity, and strengthen the
individual’s ability to work with the community and community leaders.

In this view, we would like to seek permission from your good office allow us to gather the following
data of your barangay as follows:
1. Spot map
2. Demographic profile of the residents
3. Economic and socio-cultural characteristics
4. Housing and environmental condition
5. Morbidity and mortality cases
6. Community resources and activities

We assure that any information that we gather about your Barangay will be treated with utmost
confidentiality and solely confined for our learning purposes only.
