Rs. 100/- Challan Copy - Should be taken one month before the expiry date, otherwise Rs. 1000/- should be paid as fine for renewal. . Form I - List of Commissioner of Oaths in the District concerned. 6. Form II - Certificate issued by Principal District Judge concerned. 7. Form III - Copy of Registers maintained by the Applicants. (should be duly verified and attested by the CAO of the District Court concerned) 8. Form IV - Receipts and Charge Fees Challan copy - Self Attested. 9, Form VI - Applicant details, (must be in Green Sheet) 10. Bachelor of Law Degree Certificate copy - Self Attested. 11. Enrolment Certificate copy - Self Attested. 12. Experience Certificate & Conduct Certificate from the Bar Association concerned with seal.13. Proceedings Copy to be furnished (issued by High Court) 14, Self Affidavit in Indian Non Judicial Stamp Paper. 15. Bar Association ID/ Advocate ID Copy - Self attested. 16. Any Complaint/ Allegations/ Vigilance Enquiry proceedings pending against the applicant to be dealt with as clause 16(a) of Commissioner of Oaths Rules 1988. 17. Any Disciplinary proceedings pending against the applicants before Bar Council of Tamil Nadu & Puduchuerry. 18.
