Clinical Reasoning Questions - Collaboration:

“Can a person with lupus donate blood?”

Knowing that this is a safety concern, how would you tell the staff nurse? Follow the CUS
method.

C I am Concerned that anyone with lupus, may not be


able to donate blood despite their willingness to do so.

I am uncomfortable knowing that she’s considering


donating her blood knowing they might be disqualified
U or rejected to aid others because of certain standard.
Patient with SLE may affect the body's ability to tolerate
regular blood donation. It's also possible that regular
blood donations could affect the severity of SLE.

This is a safety issue, because there are potential risks


even if she’s taking such medication since people with
S lupus have an extremely sensitive immune system.
Despite the fact that different blood banks have varied
eligibility rules, the majority of them do not accept blood
from people who have lupus.

Health teaching is a vital role for a nurse, since it can
help to prevent, manage and eliminate some diseases. In
NURSING ACTION regards to the concern of the client it is important to
educate him/her about the condition. I will tell her about
the disease in a way that he/she can understand and
explain why he/she cannot donate blood due to having
that kind of condition. Since Lupus patients have
extremely sensitive immune systems, and donating blood
is not recommended as antibodies detected in the blood
are the main concern for donating blood. It is about the
risk to the blood recipient and the possible adverse
effects of donating blood to the donor. Therefore If the
patient is related to lupus disease, donating blood can
make the condition worse. It can also make heart
problems related to lupus worse.

The ethical principles that are applicable in this situation:

ETHICAL PRINCIPLE • Respect for patient’s autonomy – Patient has the


UPHELD right to ask question in regarding about their condition
and we must also respect their decision however as a
nurse we must educate them the right decision that they
must follow.

• Beneficence — As a nurse, It is our responsibility


to do good to our clients by prioritizing their needs and
concern. The concern of the client is very important since
dangerous situations could occur. So the best thing to do
is to educate him/her about the importance of
understanding about the disease and why patient with
such condition cannot donate blood.

• Nonmaleficence — Us nurse's, we must not do


harm to our patients since it is our responsibility to give a
high quality of care. We should always ensure their
safety by preventing any dangerous situations that could
occur such as educating them about the precautions of
donating blood with lupus.

● implement the use of assistive devices for ADLs such
as:
- Long-handled sponge for bathing

- Long-handled grabber
● Teach energy conservation methods.
● Evaluate the patient’s description of fatigue: severity,
changes in severity over time, aggregating factors or
alleviating factors
● Promote sufficient nutritional intake
● Administer medication prescribed by the physician
