II. Discharge Plan


MEDICATIONS
Take medicine exactly as directed. Instruct patient to take medications at right
time, dose, frequency, route and explain when and why should take them.
- Paracetamol tablet (to relieve pain and fever if it comes back)
- Atorvastatin (to help lower "bad" cholesterol and fats and raise "good"
cholesterol in the blood)

Instruct to contact the healthcare provider if she thinks her medicine is not
helping or if experiencing side effects.

Advise patient to carry the medicine list with her in case of an emergency.

EXERCISE
Exercising helps release endorphins in the body, which are potent at
combating the feelings of sadness, anxiety or depression that might set in,
following the loss of the baby. Start with low impact activities such as
- Walking: can help clear the patient’s mind and return to a space of
calm and as well as in peace
- Yoga: can help in regaining back the flexibility of the body

Encourage patient in having enough rest. Just like exercise, the body gets the
time it needs to recover the lost strength and to heal fully.

Remind patient to avoid activities that put a lot of stress into the body like
strenuous activity or heavy lifting.

TREATMENT
Explain to the patient and significant other that their poor compliance to the
prescribed Azithromycin drug can lead to damage of the reproductive system
resulting in difficulty for a woman to get pregnant.

Encourage patient to engage in fertility massage therapy, this can assist


healing from emotional trauma. Few benefits of this are it promotes hormonal
balance, helps reduce stress, and it increase proper circulation to the uterus
and cervix.
Advise to call the health care provider if:
- Experiencing heavy bleeding and soaks through more than 2 maxi
pads per hour for more than 2 hours
- Cramps that are getting stronger and are not helped by pain
medication
- Temperature is higher than 38.3 Degrees Celsius
- Feeling of extremely weak or dizzy
- Having foul-smelling drainage or pus coming from the vagina
- Having extreme sadness, grief, or feel unable to cope with what has
happened.

HYGIENE
Educate on how to do proper perineal care regularly, to promote comfort and
cleanliness as well as increase sense of wellness.

After a pregnancy loss, breasts may still produce milk. If experiencing breast
discomfort. Instruct patient to wear a well-fitting bra with good support and
apply cold compresses for comfort.

After having D&C surgery, educate patient that she may experience having
mild cramping and light spotting for a few days and to prevent it from getting
infected, instruct that she should to avoid sexual intercourse, swimming pools,
douching, and tampons for the amount of time recommended by the
physician. Instruct to be more alert to any fever or pain, which can both be
signs of infection.

Instruct to not soak in a hot tub or bathtub for two to four weeks but may take
shower instead.

OUTPATIENT
Instruct client to visit physician on the dates given for follow-up visits, this is
for the physician to still monitor the progress of the therapeutic intervention
availed by the client. Another appointment will be scheduled for the standard
six-week postpartum exam.

Explain that the physician will let her know when the results of laboratory tests
(if any) are available. A blood test will ensure that the hormone levels have
normalized.

Suggest in writing down questions/ concerns so she can remember to ask


them during visits.
DIET
Educate that in achieving an effective result in exercise, it should be
combined with a balanced diet. Encourage to consume food that will boost the
energy levels such as
- Iron-rich foods: lean red meat, chicken, beans, green leafy vegetables.
Combine it with foods that are rich in vitamin C that helps in better
absorption of iron by the body.
- Calcium-rich foods: milk, soya
- Fruits like papaya, strawberries and grapefruits.

As abortion can lead to hormonal imbalances avoid foods that can aggravate


your condition. Avoid junk foods, sugar-based drinks and foods, and
skip food items that can cool the body down like potatoes, raw bananas, bottle
guard. Avoid fatty milk and meats such as
- Lamb, pork, beef, butter, cheese, raw meat

Encourage in adequate fluid intake. Staying hydrated is essential for fast


recovery from abortion.

SEXUAL ACTIVITY
Advise that sexual intercourse will be resume after two to four weeks, to
prevent any complication to occur such as blood clotting, inflammation and
scarring.

SPIRITUALITY
Provide spiritual and emotional support. Encourage family member to
provide patient emotional support, to lessen anxiety and stress felt by the
patient.
Abortion counseling can help patient learn to use her own inner wisdom to
heal and resolve life’s challenges.
