Occupational Safety and Fire Investigation:


Company ABC (M) Bhd is a manufacturing plant. In the process of Mr. Chandran
transporting toxic chemicals with a fork-lift, the chemical drums fell and the chemicals
flowed out onto the factory floor. The fork-lift machine involved in the incident was rented
from AKT Company. The mentioned fork-lift was not installed with any safety devices such
as alarms. Mr Chandran is a contract worker who has just started work for only one week at
AKT Company; and had not attended any safety induction courses whatsoever.
The toxic chemicals and other types of chemicals were imported from an African country via
their agent in Thailand. The chemical containers were not attached with material safety data
sheet (MSDS).
A welding maintenance job carried out nearby created a spark which initiated a fire from the
spill chemicals in the factory. The contract welding staff from WEL Engineering has been
commissioned to carry out maintenance work by the ABC Company’s Factory Manager.
However, no ‘Permit to Work’ directive for this job was ever issued by the Senior
Management of ABC Company.
The whole factory was burned to the ground in 50 minutes. Mr. Fadzil, the Production
Supervisor, was injured from the fire since he did not get out of the burning factory in time.
He did not know how to put out a fire even though he has been working in this factory for 15
years.
Ms Zeti, a factory worker, in a state of panic due to the fire injured her hand in a production
equipment at her work place. She is a temporary staff waiting for her SPM examination
results.
Mr. Pairat, a foreign general worker, tried to offer first-aid to Ms. Zeti’s hand however he
used the wrong medicine thus making the wound of Ms Zeti’s hand even worse and more
painful. Mr. Pairat was never trained in First-Aid. This is the first fire at the manufacturing
plant. The incident occurred on Sunday, March 15, 2020 at 10am.
Answer ALL questions.
a) Based from the accident at Company ABC (M) Bhd investigate any FIVE (5) root
causes of the accident.

Based on the accident at Company ABC (M) Bhd\five root causes of the accident is because of
The chemical barrels were not securely sealed, allowing the chemicals to escape due to the
forklift's fall. The drums' sealing strength should have been adequate to sustain the impact. The
forklift in question was not equipped with any safety features, such as alarms. Furthermore, the
forklift operator Mr Chandran had not attended any safety induction courses whatsoever. No
training was imparted to him before he was allowed to work in the factory. Moreover, the
chemical containers did not have a material safety data sheet attached to them (MSDS). Without
an MSDS sheet, such compounds should not be imported. A Material Safety Data Sheet (MSDS)
is a document that contains information about the chemical product's possible dangers (health,
fire, reactivity, and the environment), as well as how to work safely with it. Other than that, A
welding maintenance job carried out in a chemicals factory is highly hazardous and no ‘Permit to
Work’ directive for this job was ever issued by the Senior Management of ABC Company.
Finally, despite working in this plant for 15 years, Mr. Fadzil, the Production Supervisor, had no
idea how to put out a fire. It's safe to presume that no one was trained in fire safety procedures.

b) Write any FIVE (5) content of investigation report about Mr Fadzil, as per OSHA
requirement.

Five content of investigation report about Mr Fadzil, as per OSHA requirement is he did not
conduct any training in Fire safety mechanisms at the factory. In spite of being the Production
Supervisor, he did not know how to put out a fire even though he has been working in this factory
for 15 year.Moreover there are no permits taken for carrying out hazardous work like welding in
the factory premises which is against the OSHA guidelines. Other Than that untrained
contractual staff were permitted to work on forklift as well as other places in the factory.Also
there are no chemical process safety guidelines were implemented in the factory. No process
hazard analysis was carried out. Lastly, Mr.fadzil should have idea about incidence and protocol
that he should immediately contact the emergency ward or many try to resolve problem after
working for 15 years.

c) As the company’s Safety Officer, offer any FIVE (5) recommendations on what needs to
be done so that this unfortunate incident does not happen again.

Five recommendations on what needs to be done so that this unfortunate incident does not happen
again is that ABC company should make sure to design equipment more effectively and there should
not be any problems. Moreover, Chemical Process hazard analysis for all activities in the factory to be
done including storage, movement, stacking on racks.Furthermore, Training of staff in their respective
fields like forklift movement, storage and putaway operators should be applied. Other than that, fire
safety training to be imparted to all staff. Moreover, permits should be taken for carrying out
hazardous work like welding in the factory premises as per the OSHA guidelines. Other than that,
every worker in the field should train in a basic first aid program so that they stop the injury from
becoming serious. ABC Company should make a list of every person and make sure every worker
should attend safety course and send worker to fields with more experience and who have attended
course regularly .Finally, imported people should make sure that chemical containers are attached
with any labelling and warning stickers on it so that people in fields make sure of warings.
