This study is an international randomised phase III study for the treatment of childhood
anaplastic large cell lymphomas
Anaplastic large cell lymphoma represents approximately 10% of the childhood lymphomas.
Identified separately in the mid-1980s, they correspond in the majority of cases to what used
to be called malignant histiocytosis (1,2).
On a clinical level they are characterised by peripheral, mediastinal or intra-abdominal lymph
node involvement, by the frequency of B symptoms and by extra-nodal involvement, in
particular skin and lung involvement (3).The diagnosis rests on morphological criteria and on
the expression in the tumour cells of the antigens CD30 (Ki-1 or BERH2), EMA and of the
IL2 receptor (4). The translocation t(2;5) (p23;q35) has been described in more than 80% of
cases of anaplastic large cell lymphoma of childhood and can currently be detected by PCR
and immunohistochemistry (5,6,7). This is found in the majority of T or null cell proliferations.
There are rare B cell proliferations in which the histology is close to that of ALCL, but in most
cases these are not associated with the translocation t(2;5) and their classification in the
context of large cell anaplastic lymphomas is questioned (2). In fact, they are now
considered as a morphologic variant of diffuse large B-cell lymphomas. One of the most
difficult diagnoses is the putative but controversial Hodgkin’s-like ALCL (2).
The results of the BFM studies on 87 patients treated with the protocols NHL-BFM-B 90
(protocols used for the treatment of B cell lymphomas) have been reported by A. Reiter in
1997 (8).
All the patients received a prephase with Vincristine, Cyclophosphamide, Dexamethasone,
then the treatment was stratified according to stage:
• stage 1 and 2 resected received 3 courses: one course ‘a’ (Methotrexate 500 mg/m2
in continuous infusion over 24 hours, Ifosfamide 800 mg/m2 x 5, VP16 100 mg/m2 x 2,
Cytarabine 150 mg/m2 x 2, Dexamethasone 10 mg/m2 x 5 and triple intrathecal
treatment-Prednisolone, Cytarabine and Methotrexate) and a course ‘b’

(Dexamethasone, Methotrexate and Cytarabine, Cyclophosphamide 200 mg/m x 5,
2
Adriamycin 25 mg/m x 2) followed by a second course ‘a’.
Since 1989, SFOP has organised 2 consecutive studies for ALCL patients (9).
• Study HM89 : January 1989 to December 1990 (18 patients)
Intensive induction treatment of 1 COP (Vincristine, Cyclophosphamide, Prednisone) and 2
COPADM (Vincristine, Cyclophosphamide, Prednisone, Adriamycin, Methotrexate) and
maintenance treatment consisting of 4 cycles of 2 courses VEM (VP16, Cyclophosphamide,
Methotrexate) and VAD (Vincristine, Adriamycin). Duration - 8 months.
• Study HM91 : January 1991 to February 1997 (64 patients)
Intensive induction treatment of 1 COP and 2 COPADM, then maintenance treatment
consisting of 4 cycles of 2 courses: VEBBP (Vinblastine, VP16, Bleomycin, Prednisone). and
sequence 1 (Methotrexate, Vincristine, Doxorubicin, Cyclophosphamide and Prednisone)
Duration - 7 months.
1.1.3. UK studies
From 1990-96, most of the British patients were treated on the NHL B-Cell protocols(10).
The majority, 41/50, were treated on the 902 protocol, equivalent to the SFOP LMB 89 group
B protocol designed for B-cell lymphoma. These patients were patients with stage III and
CNS negative stage IV.
Intensive induction treatment was with COP and 2 COPADM. This was followed by 2 CYM
(Methotrexate, Cytarabine) and a final COPADM. The duration of the treatment was about 5
months. Total dose of Adriamycin 180 mg/m² and of Cyclophosphamide 4.8 g/m².
The AIEOP protocol LNH 91 consists of an induction and consolidation similar to those of
the protocol LSA2L2 of Wollner, followed by a maintenance treatment consisting of 7 cycles
of 4 weekly courses given alternately:
• Cyclophosphamide-VP16
• 6MP - Methotrexate
Among the 235 patients included in the study, 206 patients (88%) achieved a CR and 49
relapsed. The relapses occurred in the year following the diagnosis for 36 patients (74% of
the relapses). The probability of survival and disease-free survival at 3 years of the whole
population was 82% [76 - 86%] and 71% [64 - 76%] respectively, with a median follow-up of
47 months.
The overall (OS) and event-free survival (EFS) appear significantly different between the
three national groups.
Several factors seem to be associated with a higher risk of failure in univariate analysis:
B-symptoms, mediastinal involvement, skin lesions, visceral involvement, high staging in the
St Jude or Ann Arbor classification, or elevated LDH. Patients with bone lesions seem to
have a better prognosis than others. Soft tissue masses do not worsen the prognosis.
Multivariate analysis adjusted for country has permitted the highlighting of three poor
prognostic factors:
• mediastinal involvement (relative risk of failure of 2.1 [1.3-3.6] p = 0.004)
1. The best results are obtained with the BFM protocol which is less intensive than the
French one, both in terms of duration and cumulative dose of Cyclophosphamide and
Adriamycin. For this reason, a plan similar to the BFM has been adopted in the
ALCL99 study for all patients.
2. The treatment should be stratified according to risk factors. For this study, patients
with skin and/or mediastinal and/or visceral involvement are classed as a high risk
group.
3. The very rare patients with a stage I completely resected may be treated with a short
chemotherapy protocol.
The trial is a multicentre international randomised trial using a factorial design. The first
randomisation concerns the dose and schedule of administration of Methotrexate and the
second, the addition of Vinblastine. After both randomisations, there will be four arms:

• Arm 1 (reference arm): B.F.M.-K2 protocol (with Methotrexate 1 g/m over 24 hours
with intrathecal),

• Arm 2: B.F.M.-K2 protocol (with Methotrexate 1 g/m over 24 hours with intrathecal)
with the addition of Vinblastine for one year
• Arm 3: B.F.M.-K2 protocol but with Methotrexate 3 g/m2 over 3 hours and without
intrathecal

• Arm 4: B.F.M.-K2 protocol but with Methotrexate 3 g/m over 3 hours and without
intrathecal and with the addition of Vinblastine for one year.
All the patients included in the study are eligible for the trial with the exception of those
children whose disease has been completely resected and patients with CNS involvement.
Standard risk patients defined as patients with no skin lesions, no mediastinal, liver, spleen
or lung involvement are randomised between arms 1 and 3 (first randomisation only)
whereas high risk patients are randomised between the four arms (both randomisations).
Randomisation is stratified according to country and, for the question of the Vinblastine, to
prognostic factors defined by the European Intergroup Study (13).
The statistical tests will be conducted in a 2-sided form.
Patients for whom the parents decline to be randomised for the methotrexate question can
still be randomised for the vinblastine question. Those refusing randomisation should be
treated on the reference arms, which are:

• MTX at 1 g/m for the first randomisation (arms 1 and 2)
• No Vinblastine for the second randomisation (arms 1 and 3)
6.4 Other

• Clinical evaluation of pubertal status
Lung involvement should be confirmed by x-ray or CT scan. If the diagnosis can be reached
by biopsy of other organs or lymph nodes a lung biopsy is not necessary.
Isolated skin lesions are not considered a high risk factor. ALCL confined to skin is extremely
rare in childhood and may represent a distinct form of the disease. Currently it is not clear
whether patients with isolated skin disease and no extracutaneous manifestations need
chemotherapy at all. Such patients should have careful staging, central pathology review of
histology and discusses with the study coordinator before adopting a “wait and see”
approach.
This probably represents a distinct biological entity, which is extremely rare in childhood.
Such patients, whether disease is resected or not, probably do not require any
chemotherapy providing careful evaluation and staging shows no evidence of disease
elsewhere. It is proposed that these patients are followed closely for evidence of progressive
disease and that they should be discussed with the study co-ordinator.
All patients receive the prephase. The randomisation occurs before course A1.

Initial CNS involvement is very rare. Treatment should be according BFM K3 or LMB89
group C according to the protocol currently used for lymphomas with CNS involvement in
each group. CNS radiotherapy should be discussed according to the age and response to
chemotherapy.
This phase of treatment will be started as soon as the diagnosis is established with certainty.

Those patients with bulky disease may be at risk of metabolic
problems associated with tumour lysis. Alkalinisation may be
necessary.

calcium, phosphorus, urea) depending on tumour bulk.

details). This begins 24 hours after start of MTX infusion.

Dose modifications:
A blood count should be carried out every 15 days and treatment stopped if the neutrophil

count is < 0.5 x 10 /l or the platelets are < 50 x 10 /l. Vinblastine is recommenced once the
neutrophil count is > 0.5 x 109/l and the platelets are > 50 x 109/l.
• The dose of Vinblastine should be reduced to 4 mg/m² in case of prolonged cytopenia
necessitating an interruption of more than 2 injections. If the next two courses are well
tolerated, the standard dose of 6 mg/m2 should be tried again.
• Neurological examination as part of clinical evaluation should be carried out monthly. It

is suggested that the Vinblastine dose should be reduced to half (3 mg/m ) if there is
evidence of symptomatic peripheral neuropathy. In this situation please contact the
study co-ordinator.
For life threatening grade IV toxicity, secondary to Etoposide, discontinue all future
Etoposide.
7.3.2 Nephrotoxicity

For patients with a GFR by EDTA clearance of less than 60 ml/min/1.73 m contact study co-
ordinator for recommendations on further courses of high dose Methotrexate.
7.3.3 Cardiotoxicity
Cardiac assessment with echocardiography should be carried out at least before treatment
and at the end of treatment. If there is other evidence of cardiac dysfunction during
treatment and a fractional shortening is < 28%, please discuss with the study co-ordinator.
Prior to starting IV Methotrexate: if SGOT or SGPT is > 20 times normal of upper limits,
contact the study co-ordinator for further recommendations. If the transaminases are
between N x 10 and N x 20 wait 48 hours and recheck to ensure that the levels are
decreasing.
In cases of hyperbilirubinaemia/liver dysfunction, the dose of Vinblastine should be
decreased. Please discuss with trial co-ordinator.
In case of recurrent seizures, which may be due to MTX or Vinblastine, the clinician should
contact the study co-ordinator for consideration of drug modification during future cycles.
In the event of symptomatic peripheral neuropathy, reduce the dose of Vinblastine by half
and contact the study co-ordinator.
All easily accessible sites of initial disease must be assessed (either clinically or by standard
planned X-ray or by ultrasound) before each course, until complete remission is achieved.
A full assessment of complete remission (including all initially involved sites) must be
performed as soon as there is no obvious site of disease. If this assessment has not been
carried out before, it must be conducted after the third course B (B3, BV3, BM3 or BMV3).
For patients with initial mediastinal involvement, CT scan (1 cm spacing) of the thorax must
be repeated. A further repeat chest CT scan is not necessary after CR has been confirmed
unless there is doubt about the mediastinum as seen on chest X-ray, or in the case of is
relapse at another site.
8.1.3 Echocadiography
Echocardiograms must be carried out before the start of treatment, and before course B3.
Two monthly clinical follow up with chest X-ray, abdominal ultrasound and imaging of initially
involved sites for the first year.
Four-monthly clinical review from the second to the third,
six monthly from the third to the fifth year,
followed by yearly follow up.
• Echocardiograms must be carried out, 3 months after the end of treatment and then
every three years
• Renal evaluation
• FSH and LH levels must be measured at the completion of puberty and any abnormality
investigated.
All patients who are eligible for the study will be evaluated and followed the same way, even
if they are not included in the trial.
9.1 Registration
All the patients will be registered in the study, regardless of whether they are eligible for
entry in the trial and randomisation. A registration fax must be sent within 5 days after the
beginning of the pre-phase. If eligible for the trial, registration and first randomisation may be
done together.
Randomisation should be obtained for eligible patients within 3 to 5 days from the beginning
of the pre-phase and no more than 3 week days before the first course A.
The main end point is the Event Free Survival (EFS), defined as minimum time from the date
of randomisation to:
Death from any cause,
Relapse,
Progressive disease on therapy (for patients who never responded to
chemotherapy the date of event will be the date of the beginning of the treatment),
Or second malignancy.
EFS will be estimated using the Kaplan-Meier method (15).
• Overall survival, defined as time to death from any cause, measured from the date of
randomisation,
• Complete remission achieved after course B3 and lasting at least four weeks
• Short and long term toxicity,
• Non-lymphoma related death and early deaths (excluding deaths occurring after second
line treatment for failure or relapse)
• CNS relapses
The number of subjects required has been calculated in order to be able to answer the
question of the impact of Vinblastine on EFS of high-risk patients.
Assuming that the 2 year EFS is about 62% with the reference treatment (data of the
European retrospective risk factors analysis), to be able to show a 2 year EFS absolute
difference of 18% between the two arms (p0 = 62%, p1 = 80%, relative risk of failure = 2.14),
with of 5 %, of 20 % and bilateral formulation logrank test, about 59 events are
necessary. Therefore, 204 patients have to be included in the analysis: 102 patients without
Vinblastine / 102 with Vinblastine. Using a factorial design with four arms, 51 patients should
be randomised and analysed in each group (16).
With this number of subjects, the power would be equal to 63.5% ( = 36.5%) to show a
difference of 15% (p0 = 62%, p1 = 77%, RR = 1.83).
This question concerns the whole population. If 400 patients are recruited in the study, with
10% of the patients not eligible for the randomisation, 5% secondarily excluded and 5% lost-
to-follow-up, 320 patients should be analysed in this second randomisation.
Even if the question can be formulated as an equivalence question, we want to be able to
demonstrate that one arm is better than the other one, with a two-sided logrank test and an
a of 5%.
We assume that the 2 year EFS is about 70% with the reference treatment (data of the
European retrospective risk factors analysis).
With 320 “randomised and analysed” patients, the power would be equal to 80% to show a
positive difference of 13.4% (p0 = 70%, p1 = 83.4%, RR = 1.96) or a negative difference of
15.2% (p0 = 70%, p1 = 54.7%, RR = 1.69). The power would be equal to 70% to show a
positive difference of 12% (p0 = 70%, p1 = 82%, RR = 1.80) or equal to 61% to show a
negative difference of 12% (p0 = 70%, p1 = 58%, RR = 1.53).
Non randomised
patients B.F.M.-K2 B.F.M.-K2 + VBL
The first step of the analysis will be to verify that there is no significant interaction for the
Event Free Survival between the two factors (mode of administration of MTX ; addition of
Vinblastine). This will be done in a Cox regression analysis (16) restricted to the patients
included in the two randomisations. The existence of an interaction is considered as very
unlikely.
To evaluate the effect of Vinblastine on the Event Free Survival, all patients randomised for
this question will be analysed. The logrank statistic will be adjusted on the variable “ MTX ”
(non-randomised / randomised « MTX 1g/m2 » / randomised « MTX 3g/m2 »), on the
prognostic factors and on the national groups.
To test for the difference and equivalence between the two modes of administration of MTX
on Event Free Survival, all patients randomised for this second question will be analysed.
The logrank statistic will be adjusted on the variable “Vinblastine” (non-randomised /
randomised « without Vinblastine » / randomised « with Vinblastine »), on the prognostic
factors and national groups.
These analyses will be performed on the whole population. However, we plan to test for the
interaction between the efficacy of the treatment(s) and two groups of countries (national
groups included in the core group -as defined in the paragraph 4.- versus other national
groups) to look for an heterogeneity between these groups. In the event of a significant
interaction, the analyses will be performed on the core group only.

The schedule of the planned interim analysis is approximate. The time of the analysis has
been estimated using the actuarial life-table of the retrospective study with a reduction of the
risk equal to 2.14 in the group “ With Vinblastine ”, considering a homogeneous recruitment
during the time, with 60 new patients per year.
Comparison of non-lymphoma related death between treatment groups by logrank test and
comparison of early deaths between treatments groups by chi-square will be done
biannually.
An alpha-spending function (19) will be used to define the nominal p-value of each interim
analysis of the toxicity to assure a global p-value of 5% taking into account the repeated
interim analysis (10-12 analyses should be done if the duration of the trial is 6 years).
If any of these tests are significant, then a full analysis will be considered.
The absolute death rate observed in each arm (and globally in the whole population) will be
compared to a reference rate in order to detect an absolute excess of toxic deaths.
In the European series of the retrospective Intergroup Study, four early toxic deaths were
observed among 235 patients = 1.7% [0.46-4.3%].
Based on this previous experience, we choose the following parameters: p0 = 1% and
p1 = 4%, with = 5% and = 1%.
This means that the risk to wrongly conclude that there is an excess of toxic deaths
(whereas the real rate is equal to p0 1%) is equal to = 5%.
On the other hand, the power to detect an excess of toxic deaths (if the real rate is equal to
p1 = 4%) is equal to 1- = 99%.
Using the Wald’s test (19) with these parameters, the boundary is :
11.1 De kinderarts
De behandelend kinderarts coördineert de verzending van het patiëntenmateriaal naar het
Lymfomen Panel (zie Hfdst. 11.3.), het cytogenetisch laboratorium (zie Hfdst. 11.5.), en de
SNWLK (zie Hfdst. 11.4.).
De kinderarts meldt nieuwe patiënten voordat met de behandeling begonnen wordt, telefonisch
(SNWLK tel. 070-3674545) bij de SNWLK aan met opgave van naam, geboortedatum,
geslacht, primaire tumorlokalisatie, initiële leukocyten- en trombocytenaantallen in perifeer
bloed, uitslag van X-thorax (longen mediastinum), lever- en miltvergroting, huidinfiltraten,
eventueel voorlopige histologische bevindingen en derhalve verwachte diagnose.
Voor een goede gang van zaken is het belangrijk dat ook de verzending van
patiëntenmateriaal vooraf telefonisch wordt aangemeld bij de SNWLK.
Op werkdagen van 09.00 - 17.00 uur, telefoon 070-3674545 (van patiëntenmateriaal buiten

De kinderarts start de behandeling volgens het Protocol ALCL 99 op basis van de diagnose
van de plaatselijke patholoog en de uitslagen van het SNWLK-Laboratorium.
De patholoog stuurt 1 HE en 10 blanco coupes naar de secretaris van het Pathologen Panel
(zie 11.3.), de laatste op APES gecoate glaasjes, alsmede een verslag van de histologische en
immunofenotypische en evt. andere relevante bevindingen, met vermelding van "SNWLK-
NHL/ALCL 99-Protocol".

Bewaren en transporteren kan gebeuren bij kamertemperatuur.

waarop tevens enkele gegevens van de patiënt moeten worden ingevuld.

11.4.3 Liquordiagnostiek
Bij de lumbaalpunctie wordt door de kinderarts, als 2e of 3e buisje, het buisje uit het
"liquorblok" van de SNWLK gevuld. Dit wordt aangevuld tot aan de streep met liquor.
Het blok wordt direct na afname per post-expresse verzonden naar het laboratorium van de
SNWLK.
NB:
- Macroscopisch zichtbare bloedbijmenging maakt het stellen van de diagnose CZS-infiltratie
onmogelijk. Erytrocyten in de cytospinpreparaten (microscopische bloedbijmenging), maken
het resultaat onbetrouwbaar.
- Indien tijdens de behandeling door de behandelend kinderarts, ongeacht het celaantal, voor
blasten verdachte cellen worden gezien in cytospinpreparaten, dient eveneens liquor naar
het laboratorium van de SNWLK te worden gezonden. Wijze van afname en verzenden als
bij de eerste lumbaalpunctie (eventueel is het mogelijk ongekleurde cytospinpreparaten naar
het laboratorium te sturen, mits deze van goede kwaliteit zijn).
- Dubieuze liquor of bloedbijmenging.
Na 1 of enkele dagen wordt opnieuw een liquormonster afgenomen en opgestuurd naar het
laboratorium van de SNWLK.
Uitslagen worden zowel telefonisch als schriftelijk doorgegeven aan de behandelend
kinderarts.
Het chromosomenonderzoek geschiedt bij voorkeur op tumorweefsel; indien dit niet mogelijk
is, kan het onderzoek worden verricht op beenmerg, bloed, liquor, pleura- of ascitesvocht, mits
dit tumorcellen bevat. Het chromosomenonderzoek geschiedt in 8 cytogenetische laboratoria in
Nederland.Voor het doen verrichten van cytogenetisch onderzoek neme men telefonisch
contact op met één der onderstaande personen en instituten:

NB. Randomisaties zijn uitsluitend mogelijk op maandag t/m vrijdag, van 8.30 tot 14.30
uur
Bij het verzoek tot randomisatie dienen de gegevens ten aanzien van eligibility door de
behandelende kinderarts aan de SNWLK te worden verstrekt (zie hoofdstuk 5).
Door de SNWLK wordt vervolgens contact opgenomen met het studiecentrum in Parijs, waar
de randomisatie wordt verricht. De behandelende kinderarts krijgt per omgaande telefonisch
en schriftelijk bericht over het resultaat van de randomisatie.
The absence or presence of fever, night sweats and/or unexplained loss of 10% or more of
the body weight in the 6 months preceding admission are to be denoted in all cases by the
suffix letters A or B respectively.
Representative tissue sections should be submitted for fixation including one block with 10%
Formalin and, if possible, a second block of tissue with a different fixative routinely used by
the local Pathologist e.g. B5, Bouins fixative, AFA, Zenker, alcohol based Bouin etc. The
fixative utilised and length of time in fixative before processing should be indicated for each
respective paraffin block. Fixation of tissue for over 24 hours should be avoided to allow
optimal antigen preservation and immunophenotypic analysis.
If sufficient tissue is available, representative fresh tumour tissue should be snap frozen at -
70oC and placed in a plastic tube covered with corkboard.
Touch imprint preparations should be made on ten (10) sialinised glass microscope slides,
fixed in 100% methanol for at least three minutes and air dried completely. These slides
should be stored frozen at -20 to -70oC. These preparations can be used for fluorescence in
situ hybridisation studies.
If sufficient tissue is available, representative fresh tissue should be submitted for
cytogenetic analysis. If a cytogenetics laboratory is not available to the submitted institution,
the fresh tissue in cytogenetics media should be sent to an alternative cytogenetics
laboratory (where available) by overnight delivery.
2.3.1 Morphology
Presence of the hallmark cell (a large sized cell with eccentric kidney shaped nucleus)
The following are recognised morphological variants of large cell anaplastic
lymphoma

• Sinusoidal growth pattern (sinusoidal infiltration)
• Perivascular pattern (malignant cells surrounding vessels; better assessed after
immunostaining with CD30 or ALK 1 antibody)

• Erythrophagocytosis by reactive histiocytes
• Nodular sclerosis
• Large number of plasma cells
• Large number of eosinophils
2.3.2 Immunophenotyping
For initial determination of cell lineage (T, B, or null) the methodology and criteria for
immunophenotypic analysis defined by the submitting institution will be accepted.
Recognised methods include paraffin section, immunohistochemistry, frozen section
immunohistochemistry, and cytospin immunocytochemistry.

4.1.1 Hydration

Prehydrate with 125ml/m /h (dextrose saline with NaHCO3 50 mmol/l) for a minimum of 2

hours in order to achieve a pH of 7 and a urine output of 100 ml/m /h.

During and after MTX infusion, continue hydration at a rate of 3000 ml/ m with NaHCO3 50
mmol/l and KCL 20 mmol/l added to dextrose saline to maintain urinary pH 7 for a further
48 hours. Methotrexate levels, urea and electrolytes should be measured daily for at least 3
days after MTX infusion. Strict attention should be paid to fluid balance.
Drugs which compromise renal function e.g. aminoglycosides can decrease clearance of
Methotrexate and lead to systemic toxicity. Avoid concurrent use of NSAIDs including
salicylates and sulphonamides. Penicillins may interfere with active renal tubular secretion of
Methotrexate. Prophylactic co-trimoxazole (if given) should be stopped one week prior to
MTX administration.
It is important to note that for this schedule the folinic acid rescue starts very late and is short
(3 doses in cases of normal MTX excretion). As the rescue is short, the folinic acid
should be prescribed i.v. so that the rescue is optimal.
MTX plasma concentrations are measured at hr 24 and hr 48 from start of MTX infusion.
The expected MTX plasma concentrations are given in the following Table. Folinic acid

rescue begins 42 hours from start of MTX infusion. 15 mg/m of folinic acid should be
given i.v. at hrs 42, 48, 54 from the start of MTX infusion as shown in the table below.
If the plasma MTX concentration at 48hrs from start of MTX infusion is <0.25 cmol/L

(<2.5x10 M), then folinic acid rescue is stopped after 54hrs and no more measurements of
the plasma MTX concentrations are needed.
If the plasma MTX concentration at 48hrs from the start of MTX is >0.25 cmol/L
(>2.5x10-7M), continue the folinic acid rescue beyond 54hrs according the schedule below.
Continue with measurement of the plasma MTX concentration every 24 hrs.

5.2 Septrin
Prophylaxis against pneumocystis carinii with Septrin is recommended for the high risk group
who are randomised to receive weekly Vinblastine as maintenance therapy. The
recommended dose is 25 mg/kg of Septrin 3 days a week. The treatment should be
continued until 3 months after the end of the chemotherapy.
5.3 Mucositis
Mucositis is expected particularly after course B. The use of mouthcare should be
encouraged to reduce oral infections and prevent mucositis.
6.1.1 Vinblastine

Administration 6mg/m given as an intravenous bolus. Max dose 10mg
6.1.2 Cyclophosphamide
Formulation 100 mg, 200 mg, 500 mg and 1 G vials for reconstitution.

Administration The dose is 800 mg/m by intravenous infusion over 60. Mesna is given
as an iv bolus330mgs/m2 at 0,4 and 8 hrs after the ifosfamide. Hydration
is also required (see individual course details for more information.)
6.1.4 Dexamethasone
Administration 5 mg/m2 daily in one dose or 10 mg/m2 by mouth orally twice daily in
divided doses.
6.1.5 Doxorubicin

Administration In this protocol 25 mg/m is given as an infusion over 1 hour. The drug
should be mixed with 0.9% saline. Prolonged contact with solutions of
alkaline pH should be avoided as this will result in hydrolysis of the drug.
Doxorubicin should therefore be infused via a separate lumen from
alkaline solutions following methotrexate administration.
6.1.6 Methotrexate
Reconstitution In general dilute to a concentration of not more than 0.4 mg/ml etoposide
in 0.9% sodium chloride or 5% dextrose. It can be made up in a more
concentrated solution (1:1) if volume is a problem, but in this case the
solution must be used immediately.
Stability Vials are stable for 5 years at room temperature. At concentrations of 0.4
mg/ml in 0.9% saline solutions are stable for 96 hours at room
temperature in normal fluorescent lighting.

Administration 100 mg/m by intravenous infusion over 2 hours - protected from light.
Caution: Anaphylactic reaction usually manifested as severe hypotension
may occur if infusion given too rapidly. Avoid extravasation.
6.1.8 Cytarabine
Toxicity Bone marrow suppression, nausea, vomiting, oral ulceration. fever and
arthralgia, diarrhoea, mucosal membrane inflammation, ulceration and
bleeding, alopecia and flu-like syndrome. For intrathecal administration,
reconstitute with preservative-free lactated Ringer's or normal saline.
Ara-C is compatible with KCl and NaHCO3.
At higher doses (3000 mg/m2) cerebllar toxicity may occur.
Gastroinetstinal toxicity with diarrhoea, mucositis and vomiting may also
be more severe. Pulmonary toxicity is uncommon, but may present with
unexplained breathlessness. Conjuctivitis can be distressing, but it may
be prevented by the regular use of prednisolone eye drops.
Reconstitution Reconstitute each vial with bacteriostatic or sterile water for injection, to
achieve a final concentration of 10 mg/ml. No data is available concerning
compatibility with KCl or NaHCO3.

INLEIDING
Bij uw kind is onlangs een zeldzame vorm van lymfeklierkanker vastgesteld. Nader onderzoek
heeft uitgewezen dat er sprake is van een Grootcellig Anaplastisch non-Hodgkin Lymfoom,
volgens de Engelse afkorting een ALCL (anaplastic large cell lymphoma). De diagnose
Grootcellig Anaplastisch non-Hodgkin Lymfoom wordt meestal bij volwassenen gesteld, maar
helaas soms ook bij kinderen. Deze ziekte kan voorkomen in de huid, lever, milt, longen of
borstkas, hersenvocht of ruggenmerg. Kinderen waarbij de ziekte op meer dan één plaats
voorkomt behoren tot de zogeheten Hoog Risico Groep (HRG). Deze informatie betreft de
behandeling in combinatie met wetenschappelijk onderzoek van Grootcellig Anaplastisch non-
Hodgkin Lymfoom in dit ziekenhuis.
DIAGNOSE
Om de definitieve diagnose te kunnen stellen wordt zeer uitgebreid onderzoek ingezet om
vast te stellen waar het lymfoom zit. Onder narcose wordt door middel van een kleine
operatie een stukje van het gezwel weggenomen om precies vast te stellen met wat voor
soort gezwel we te maken hebben: de diagnose wordt gesteld. Verder wordt tijdens
diezelfde narcose bloed, beenmerg en hersenvocht afgenomen om eventuele uitbreiding van
ziekte te registreren. Er zullen ook altijd foto’s gemaakt worden, waarvan die foto`s gemaakt
worden hangt af van waar het gezwel zit. Als de diagnose eenmaal gesteld is, wordt zo snel
mogelijk met de behandeling begonnen. De behandeling van uw kind hangt af van de plaats
van het lymfoom en de uitbreiding hiervan.
BIJWERKINGEN
Bijwerkingen van chemotherapie kunnen per medicijn verschillend zijn. De behandelend arts
zal daar in een gesprek nader op ingaan. Meer over bijwerkingen van chemotherapie kunt u

Indien u besluit geen toestemming te geven voor deelname van uw kind aan deze studie zal dit

INLEIDING
Bij uw kind is onlangs een zeldzame vorm van lymfeklierkanker vastgesteld. Nader onderzoek
heeft uitgewezen dat er sprake is van een Grootcellig Anaplastisch non-Hodgkin Lymfoom,
volgens de Engelse afkorting een ALCL (anaplastic large cell lymphoma). De diagnose
Grootcellig Anaplastisch non-Hodgkin Lymfoom wordt meestal bij volwassenen gesteld, maar
helaas soms ook bij kinderen. Deze ziekte kan voorkomen in de huid, lever, milt, longen of
borstkas, hersenvocht of ruggenmerg. Kinderen waarbij de ziekte op één plaats voorkomt
horen tot de zogeheten Standaard Risico Groep (SRG). Deze informatie betreft de
behandeling in combinatie met wetenschappelijk onderzoek van Grootcellig Anaplastisch non-
Hodgkin Lymfoom in dit ziekenhuis.
DIAGNOSE
Om de definitieve diagnose te kunnen stellen wordt zeer uitgebreid onderzoek ingezet om
vast te stellen waar het lymfoom zit. Onder narcose wordt door middel van een kleine
operatie een stukje van het gezwel weggenomen om precies vast te stellen met wat voor
soort gezwel we te maken hebben: de diagnose wordt gesteld. Verder wordt tijdens
diezelfde narcose bloed, beenmerg en hersenvocht afgenomen om eventuele uitbreiding van
ziekte te registreren. Er zullen ook altijd foto’s gemaakt worden, waarvan die foto`s gemaakt
worden hangt af van waar het gezwel zit. Als de diagnose eenmaal gesteld is, wordt zo snel
mogelijk met de behandeling begonnen. De behandeling van uw kind hangt af van de plaats
van het lymfoom en de uitbreiding hiervan.
BIJWERKINGEN
Bijwerkingen van chemotherapie kunnen per medicijn verschillend zijn. De behandelend arts
zal daar in een gesprek nader op ingaan. Meer over bijwerkingen van chemotherapie kunt u
lezen in de dagboekagenda die u aan het begin van de behandeling krijgt. Er wordt tijdens de

of op meerdere plaatsen. Nadat jij in slaap bent gebracht (onder narcose) wordt door middel

met wat voor soort gezwel we te maken hebben: de diagnose wordt gesteld. Verder wordt
tijdens diezelfde narcose bloed, beenmerg en hersenvocht afgenomen. Er zullen ook altijd

standaardbehandeling, die hetzelfde is als arm 1 van de randomisatie.
Je krijgt voldoende tijd om samen met je ouders over de randomisatie na te denken, er worden
immers pas na de eerste kuur keuzes gemaakt. Je kunt altijd om extra uitleg vragen of op
eenmaal genomen beslissingen terugkomen, zonder hiervoor een reden op te geven.

b) Each national group Data Centre will be responsible for distribution of protocols to centres
within that national group.
c) Addenda may be added independently by any of the national groups to address local
needs, provided they have no bearing on the essential aims of the international protocol.
d) Subsequent to finalisation, any amendments to the protocol must be agreed by all the
national groups. The Central Data Centre, as the co-ordinators of the protocol, will issue a
revised version of the protocol, as required.
a) Data may be collected on one common set of forms and forwarded to the central data
centre or data may be collected in each national data centre and sent electronically using
a common coding system, format and identical cross-checks.
b) Each national group Data Centre will be responsible for distribution of forms to centres
within that national group.
c) Additional forms may be produced independently by any national group for the collection
of data additional to that required for the international study.
d) Subsequent to finalisation, amendments to the forms must be agreed by all national
groups. The central Data Centre will be responsible for the issue of amended forms.
e) Each national group shall collect forms for its own patients, and shall be responsible for
data quality according to local practice. The data will be send at least 6-monthly.
f) Each national group will decide whether the registration and two randomisations will be
carried out in their own Data Centre or send by fax to the Central Data Centre.
g) The master database for the entire study will be held at the SFOP Data Centre.

a) The use of names as patient identifiers on paper forms and on national databases will be
according to national practice.
b) An abbreviated patient identifier may be used for data transfer and for the master
database.
a) On receipt of forms at the national data centre, common range and logical checks will be
carried out on data prior to transfer to the master database. Any amendment to the
checking programs shall require mutual agreement of all national groups.
b) Data verification shall be carried out according to current national practice. Cross checks
of data entry will be carried out occasionally, between national centres, on a sample of
forms.
c) Errors noted on the master database, after receipt of the group data, shall be reported
back to the national centre.
a) Results of the interim analysis of outcome and toxicity shall be reported to an independent
international data monitoring committee (DMC) as scheduled by protocol. The DMC may
recommend early stopping, continuation or extension of the study to the international
study committee.
b) Reports prepared from the national databases on overall outcome may be circulated
locally subject to approval for release by the DMC. No other publication will be allowed.
a) Any adverse event (death, relapse or Grade 4 non-haematological life threatening toxicity)
shall be reported immediately by the treating institution to the national centre and relayed
to the other national centres, for further reporting according to local practice.
b) The toxicity criteria will be the same for all participating groups and appear in Appendix 4.
a) For all cases, diagnosis is based on local pathologist observation but slides must be
reviewed by one of the pathology panel.
b) Cases not fulfilling the classical criteria for diagnosis (typical histopathology,
immunochemistry: CD30+, EMA+, NPM-ALK+ (if available), null or T-immunophenotype)
have to be reviewed by the national pathologist before the second randomisation.
c) Special cases, including those with atypical immuno-phenotype, will be reviewed by the
whole pathology board.
d) National group pathology data shall be discussed at the meeting of the international study
committee. This pathology committee will meet as appropriate.
