Graphical User Interface/Module
An initial layout or design of the proposed system must be created during the
system design process in order to envision and understand the proposed system
better using the visual space available. The proponents developed an initial
graphical user interface for the proposed method, as is seen in the following
figures.

In this figure it has two buttons containing login as admin that goes to the
admin form and start check that goes to the patient form.

In this figure you will input your username and password. Once the details
are correct it goes to the admin form, if the details are not matched it will be
incorrect and it will not go to another form.

In this figure it has a picture of a human and contains a lot of buttons which
have their own function (View Symptoms, View Disease, Add Symptoms, Add
Disease, Add Admin, Admin Checkups, Feedback, View Users, Change
Password and Change Username). The function will be in the following figure:

In this figure you will see a lot of symptoms that can survey the patient's
problem. As an admin it can edit the symptoms and it can also delete.

In this figure you will see a lot of disease names that can help the patient's
problem to see the result of their check up. As an admin it can edit the disease
name, cause and it can also delete.

In this figure adding the number of input id, symptoms name and selecting
a disease that match on the symptoms.

enter the cause disease that match on the disease.

In this figure is the registration of another admin to access the admin form. It
contains username, password and confirm password. In this registration is to
avoid missing person work in the center.

In this figure it's a private check up for admin once you choose a part of
the body like nose it will go to the symptoms of nose disease and you have a
check for answering the survey by yes or no and last the website will give your
result based on the symptoms that you answer.

In this figure is the user of the website. Once the user answers all the surveys
in the online checkup and shows his result it has a button feedback that input
name, how experiencing? Send messages and input email. The admin can
remove it if he wants.

In this figure is the user information. Once the user registers, uses the
system and goes for an online check up the admin can notice it by username,
contact, registration date and result of the check up.

In this figure is an updating admin password. Once they want to change

your password input the current password it means your previous password.
New password and confirmation of new password.

In this figure is the home page of the patient that the main button contains
register and login form. Inputting the details required to go in another form of the
website.

In this figure you will input your username and password that include
uppercase, lowercase and number. Email for confirmation. Once the details are
input registration will be successful.

In this figure once you are registered you will input your username and
password once the details are correct it goes to the patient form, If the details do
not match it will be incorrect and it will not go to another form.

In this figure is the online checkup for patient once you choose an part of
the body the head, ear, nose, eyes, neck, shoulder, chest, arm, elbow, forearm,
abdomen, wrist, pelvis, pubis, hand, thigh, knee, leg ,ankle and foot to start the
online checkup .

In this figure is the start of online check up the health worker provides a lot
of surveys of symptoms for the patients. Answering the survey by yes or no.

In this figure once the patient answers all the surveys of symptoms it
analyzes the data’s and gives the result, suggested disease and cause.

In this figure is the patient feedback form. The patient will input the name,
how experienced the website is (bad, good and average), send comments, input
name and email.

In this figure is an updating patient password. Once they want to change

your password input the current password it means your previous password.
