Submission of Application

1. Log in to r4b.lms.deped.gov.ph, select your Division

2. If you do not have an LMS account, click "DepEd Commons Teachers"

3. In the Commons log in page, click on "Continue with Google Account" and log in your DepEd
account

4. In the DepEd Commons website, click the “Authorize” button to continue

5. In your LMS Dashboard, click on "Site Home"

6. In the Course categories, click on "Professional Development"

7. Click on the "Facility for the Submission of the Application Requirement for the PNPKI Digital
Certificate of DepEd Personnel in the Field Offices"

8. Enter the Enrolment key for our Division: PNPKI_PuertoPrincesaCity1

9. Click the "DOWNLOAD THE PNPKI FORM"

10. Locate the downloaded PDF file and open it with Adobe Reader DC (Adobe Reader DC may be
downloaded and installed for free directly from the internet); accomplish all the required details of the
form correctly (Note: Please make you have a softcopy of your Passport size ID picture and signature)

11. On the LMS site, click on the topic “(Your Division)”

12. In the Activities, click on "PNPKI Questionnaire", click the “Attempt quiz” button and answer the
questions correctly; Click "Submit all and finish"

13. Click on "Upload PNPKI Form Here" and upload the accomplished PDF form; Click on "Save
changes"

14. The submission will be graded by the Division PNPKI Team, and a notification will be sent if there
are any changes needed in the submission

Note:

a. Applicants with corrections can see the status of their application if completed. If there are
corrections, find Add a new attempt based on previous submission and make revisions or re-upload
corrected PDF file.
