WAIVER/QUITCLAIM

KNOW ALL MEN BY THESE PRESENTS:

I, JENNIFER C. HINGPIT of legal age, Filipino, single/married and a resident of Makilala,

North Cotabato, for and in consideration of the sum of PESOS:SEVEN THOUSAND EIGHT
HUNDRED EIGHTY ONE & 04/100 (P7,881.04), receipt of which is hereby acknowledged, do
hereby forever waive, release and render harmless HOLY CHILD COLLEGE OF DAVAO (HCCD) and
any of its officers, agents and assigns, from any and all causes of action, claims and demands, for
separation pay, retirement pay, salaries, wages, overtime pay, 13 th month pay, service incentive,
holiday pay, or any other fees, SSS and Pag-ibig premiums and for any and all claims and liabilities
which I may have against the said educational institution.

The aforesaid amount represents the full amount which I am entitled to receive from the said
company.

I bind myself not to pursue any future claim with any tribunal, judicial and/ or quasi-judicial
agency, and/or lodge any complaint against the said company.
