Chancroid
Chancroid
a. (Chancroid)
b. Pathogen
Chancroid is caused by the bacterium Haemophilus ducreyi and results in painful, superficial
ulcers, often with regional lymphadenopathy. Chancroid occurs in Asia, Africa, and the
Caribbean, and is an important cofactor of HIV transmission. The genital ulcer from chancroid is
painful, tender, and nonindurated.

c. Prevalence (age, gender, indicate the general population affected either worldwide or in
the Philippines data must be at least 2018 and above)
UNAIDS and the World Health Organization estimate the global incidence of chancroid to be
approximately 6 million cases per year.
Chancroid may develop in individuals of any age but is typically found in sexually active
individuals with a mean patient age of 30 years.
The male-to-female ratio of patients with chancroid ranges from 3:1 in endemic areas to 25:1
during outbreaks situations.
Female sex workers with either symptomatic chancroid or as asymptomatic carriers serve as
a reservoir for H. ducreyi.

Most people with chancroid begin to notice symptoms between 3 and 10 days after
contracting the infection.
The most common symptoms of chancroid are painful, red-colored bumps in the
genital region that become ulcerated, open sores.
Chancroid sores are often very painful in men but less noticeable and painful in
women.

e. Adverse Effect
Chancroid causes ulcers, usually of the genitals. Swollen, painful lymph glands, or inguinal
buboes, in the groin area are often associated with chancroid. Left untreated, chancroid may
facilitate the transmission of HIV.

f. Mode of Transmission
g. Diagnosis (Test Available)
Diagnosis is made by isolating the bacteria Hemophilus ducreyi in a culture from a genital ulcer.
The chancre is often confused with syphilis, herpes or lymphogranuloma venereum; therefore,
it is important that your health care provider rule these diseases out. A Gram stain to identify H.

h. Treatment Option
The recommended first line therapy for chancroid according to Centers for Disease Control and
Prevention (CDC) STD treatment guidelines is one of four regimens: azithromycin 1 g orally in a
single dose, ceftriaxone 250 mg intramuscularly in a single dose, ciprofloxacin 500 mg orally 2
times a day for 3 days, or erythromycin base 500 mg orally 3 times a day for 7 days. Treatment
regimens for chronic ulcers in children caused by H. ducreyi have not been studied, hence
recommended antimicrobial treatment options at this time cannot be
recommended. Nevertheless, it appears that single-oral-dose azithromycin 30 mg/kg may be
effective in children.

i. Prevention – must be aligned with Health Sector Plan for HIV and STI 2015-2020
STRATEGY 1: Continuum of HIV/STI prevention, diagnosis, treatment and care services
to key populations
The only sure way to prevent chancroid is to avoid all sexual activities and contact.
However, total celibacy is not a realistic lifestyle choice for the majority of people.
