WEEK 2 RLE ACTIVITY

CASE SCENARIO1:PAMILYA BETOBETO. The Betobeto Family lives in Sitio San Ysidro,
Barangay San Jose. 18\7-year-old Zaide is a farmer and his16-year-old wife Juanita,is a
laundry woman. She is pregnant with their first child. Her prenatal care is done at
the Rural Health Unit (RHU). A midwife reports once a week, at which time, the RHU
opens. Juanita’s mother Julia supervised her pregnancy and ensured that she takes
nutritious meals and vitamin supplements, as well as the required laboratory exams
were done; her maternity booklet was also updated during her prenatal visits by the
midwife. A primary hospital is located three hours away from where the family
lives; a private Maternity Clinics also located a few kilometers away from the said
hospital. On her sixth month of her pregnancy, the anti-tetanus injection was refused, as
her mother believes she was too young for it.; the midwife advised them to undergo
an ultrasound and to start preparing for Juanita’s admission to a bigger hospital in
Marikina, should circumstances during delivery require additional procedures. Aling
Julia, for her part considers taking her daughter to the maternity clinic, which is nearer,
when her time to deliver comes.

Questions:

1. Describe the level of care that is in Sitio San Ysiro.


• People in rural areas like Sitio San Ysiro, generally have less access to healthcare
than their urban counterparts. Fewer medical practitioners, mental health
programs, and healthcare facilities in these areas often mean less preventative
care and longer response times in emergencies. They often encounter barriers to
healthcare that limit their ability to obtain the care they need.
2. What would be the nature of intra-natal care that Juanita will need.
• Intra natal care means care taken during delivery. This consists of taking care of
not only the mother but also the newborn at the time of child-birth. On the case of
the patient, appropriate delivery care is crucial for both maternal and perinatal

health. Increasing skilled attendance at birth is a central goal of the safe


motherhood and child survival mission. Postpartum care aimed at complication-
free puerperium and a healthy baby. There is a high risk of mortality for both the
mother and her baby in the immediate period around birth. The nature of intra-
natal care that Juanita will need is a high quality intra-natal and postnatal care to
reduce the maternal morbidity and mortality. Utilization of intra-natal and
postnatal services is poor in the rural areas, which ultimately lead to increased
maternal and child morbidity and mortality, that’s why the midwife advised them
to undergo an ultrasound and to start preparing for Juanita’s admission to a bigger
hospital in Marikina to ensure that the care taken before, during and after the
delivery is protected from danger or risks both the mother and the child.
3. Contrast the available health resources and required health resources for
Juanita and her family.
• The availability of health resources in their community is insufficient. A lack of
resources in healthcare settings has serious consequences for the quality of
patient care and the professional work environment for nurses, therapists, and
other healthcare providers. Having more health system resources available and
making better use of the resources are two approaches that can suffice the needs
of workers and meet patients’ expectations. Appropriate delivery care is crucial

With this commitment to save mothers and newborns by providing them financial
risk protection, there is a need to redefine PhilHealth's maternity and newborn
care which was established to provide health insurance coverage and ensure
affordable, acceptable, available and accessible health care services. These
benefits will focus on the health services that the pregnant women must receive
throughout their pregnancy and delivery. Also, PhilHealth shifted its provider
payment mechanism to case-based payment which aims to increase efficiency in
health care provision, simplify understanding of PhilHealth benefits by all sectors
and improve the process of availing them.

5.What does Pamilya Betobeto show us about the health care delivery system.
Illustrate.

• Pamilya Betobeto show us about the health care delivery system in rural areas are
often deficient in human resource, infrastructure structure, equipment, and
financial support. Fewer medical practitioners, mental health programs, and
healthcare facilities in these areas often mean less preventative care and longer
response times in emergencies. All of these factors can lead to poor
health outcomes.
