CHAPTER ONE:

GENERAL INFORMATIONA OF THE PROJECT

1.1 General Information

Mio Health Centre Staff Residence Construction Projects located the south eastern part of Bale
in the Dawe Kachen district and 135 Km from Robe town. The Employer of this project is Bale
Zone Health office. The project price is estimated to ETB 3,939,437.77 including all local taxes.

The whole project consist construction of 3 blocks (10 rooms) of residence room and 1
traditional kitchen with shower and toilets.

1.2 SCOPE

This part covers all the processes, techniques and methodologies to develop the project
construction plan which aims at the execution and completion of the Construction Mio Health
Centre Staff Residence Construction Projects in Bale Zone works within 120 working days.
1.3 SUMMARY OF INPUT DATA FOR PLANNING
This part covers the collection and formulation of data / information which is used to prepare the
work programs and schedules.

CHAPTER TWO:

WORK METHOD STATEMENT

This is to explain the arrangements and methods proposed to adopt for the
execution of Mio Health Centre Staff Residence Construction Project on the basis of
the scope of work outlined in the bill of quantities, contract drawings and
specification thereof. Accordingly, the work program consists of a detail work
method statement of each particular construction activities by their category or
type and the requirements of different resources for each of particular activities
i.e., manpower, equipment and machineries, materials, etc. to achieve the
completion of the construction in accordance with the given contractual
obligations. To make it compatible with actual changing circumstances, delays or
amendments to the work ordered by the Engineer, this method statement is
subjected to be reviewed in due course of time.

Furthermore, it is attempted to incorporate the basic scheduling information


including formulation of work crew, average daily crew production estimation,
work break down structures of each tasks, equipment availability and utilization
factors and a general description of our proposed arrangements and methods for
the execution of the Works.

2.1 General and Preliminary Operations

The main operations included in this part of the project phase, but not limited, are
staffing and mobilizing Contractor’s key personnel to the site which have been
organized as Mio Health Centre Staff Residence Construction Project Team and
backed with experienced personnel. The investigation and assessment of the
availability of the required quantity and quality of the necessary labor and local
construction materials i.e., sand, water, stones, gravels etc., will be undertaken
during this stage. As usual, all material to be incorporated in works and
workmanship will be strictly of the respective kinds described in the contract and
in accordance with the Engineer’s instructions. In order to produce work which
conforms in quality and accuracy of dimensions to the requirements of
specification and drawings, a Quality control system with experienced staff,

together with all necessary facilities to ensure adequate supervision and positive
control of the works at all times ,will be implemented at site

We also study the duration and intensity of the rainfall by collecting data from
local villagers, work schedule reflects the prevailing weather condition.

The aforementioned lists of operations are supposed to be accomplished prior to


the commencement of all other main works so that we can establish smooth and
effective working backgrounds.

As set out in drawings and BOQ, the principal items of work to be executed under
this contract are likely to include but are not limited to the following:

 Site Clearance and Setting out;


 Earthworks;
 Stone Masonry work;
 RC steel work;
 Concrete work;
 Block work;
 Wall and Floor finishing work;
 Pavement and drainage work;
 Sanitary work;
 Electrical Work;
 Taking care of the Works prior to the issue of a Taking-Over Certificate and
rectification of defects during the Defects Liability Period of 365 days.

Construction procedure for each of the above mentioned activity is briefly presented on
following pages:

2.2 Work plan and site organization


The intent of this construction work plan and site organization proposal is to
present sequence of construction activities, various phases of work , possible
utilization of personnel, equipment, materials, site utilities and combination of
these resources which are planned to take place at the site.

2.2.1 Mobilization
The mobilization of the project facilities, manpower, and light and heavy duty
equipment’s will be done step by step following the construction work schedule,
which gets earlier approval by the client, from their existing locations in order to
meet the proposed construction schedule. The necessary construction materials
required for the construction contractor’s temporary facilities: - site offices,
material stores, equipment stores, and guard house will be supplied at appropriate
location,

2.2.1.1 Mobilization of equipment

The required equipment for this particular project will be mobilized as per the
proposed equipment mobilization schedule attached here in with.

2.2.1.2 Mobilization of manpower

The necessary skilled and unskilled manpower will be deployed to the project site
as per the proposed schedule. The site organization and the assignment of key
personnel assigned is acquired by the contractor, this will be communicated to the
consultant and the client, and such change will only be effected if and only if
approval is given from the consultant and the client.

2.2.1.3 Mobilization of construction materials

High quality cements, reinforcing bars, sand stones, nails, and other construction
materials will be delivered after sample approval of the assigned engineer. Also

other construction materials will be delivered to site according to the delivery


schedule to the proper quality and quantity and stored at appropriate places.

2.2.2 Site Organization


The site will be organization in such a way that we can meet the performance,
quality of construction and targeted time.

2.2.2.1 Construction of material production shade

Considering the constraint of space within the vicinity of construction site, the
required amounts of bar benders shade and other necessary shades will be
constructed on this stage.

Every of the above step will go through all the necessary test processes and will be
ready for use after the approval is granted by the resident engineer.

2.3 Major Construction

2.3.1 Site clearance & earthwork

2.3.1.1 Setting out

Prior to the start of the execution of the works, a survey will be executed arranged
by the contractor showing the condition of the existing and adjacent structures.
The location of any defect found will be determined and described. The result is to
be presented to the Engineer .We will appoint and employ the necessary qualified
and experienced staff to set out the work accurately including all necessary
chainmen. It will set out the works well in advance and in sufficient time to enable
the engineer to inspect and approve prior to commencement of the works.

Topography survey of the site will be performed by a senior surveyor and will
check the accuracy of the levels shown on the drawings or the directions given by
the engineer. This will establish an approved primary bench mark and a base line
on which all subsequent setting out will be based. Following this and prior to
construction activities, job/sit/ layout will be prepared and temporary facilities
construction will be commenced. Setting out will be done at every structural level.

The construction begins first by organizing the necessary work crews and then the
first activity will be to locate exact position of block as per the detail plan and
sections using surveying equipment. After locating the positions, the construction
work starts from earth work.

2.3.1.2 Excavation and earthwork

All excavation will be carried out to the lines, levels, widths, depths and grades shown on the drawings or

as directed by the engineer and as appropriate to the works to be placed in the excavation.

We shall examine the site carefully and will ascertain the nature and kind of
materials to be excavated.

Faces of excavations will be retained with sheeting, timbering, strutting and


shoring as necessary to protect workmen and to prevent damage to adjacent work,
services or structures. Design and construction work for the shoring work will be
executed by specialists.

Surplus excavated material will be removed from time to time to the appropriate
dumping ground.

2.3.1.3. Fill material

Fill to excavations or to make up levels will be made in suitable material approved


by the engineer either from site or from borrowed material imported from quarry
sites. Compaction will be carried out with rollers and walk behind compactors
after applying the necessary moisture by sprinkling water and to the specified lift
thicknesses. The final levels of fill will be adjusted, graded and prepared to receive
bedding to be laid on the fill.

2.3.2 CONCRETE WORK


2.3.2.1 Cement
- We are responsible for supply of binding material and the cement used for all
water tight structures and it will be best quality cement approved by the
responsible engineer, the quantities of cement for the work will be sufficient to
ensure the proper progress of the works and sub stores) will be raised above
the ground level to prevent the absorption of moisture.
- We use the cement as soon as possible after delivery and stored not longer than
four months.
2.3.2.2 Water
- The water we use for making all works like concrete and mortar will be from
river water from stream treating it with disinfecting chemicals which we will
make it free from significant quantities of silt, organic matter, alkali, salt and
other impurities after approval of the engineer. So than the impact on the
chemistry of concreting will be minimized.
- Test for water will be conducted for PH, turbidity, salt and sulfate content in
the presence of the engineer and client.

2.3.2.3 Gravel

- Gravel used for concrete works will have a size of 20mm which is prepared and
produced by crusher. The stone for production of gravel will come from
quarries found around Adama city. For testing the stone whether it is basaltic
or not, a prior tests conducted by compressive strength machine will determine
the approval of this material by the engineer.

2.3.2.4 Sand

- Sand (fine and coarse) will be supplied from quarries found around Adama city
which will approve by the engineer, but before supplying, test will be
conducted. The gradation and cleanness will be as per the standards given in
the contract document.

2.3.2.5 Form work

Prior to every concrete activity formwork placing will be performed carefully and
will be prepared for the next day so that concrete will be filled. Before every
concreting step the construction engineer will check for its appropriate erection
and will give an order for the general Forman to proceed. This step is critical for
best concrete results and for structural reasons. Needed materials will be
delivered in bulk to the site during mobilization this includes ply wood, timber,
eucalyptus, nails and other necessary tools. Well Experienced man Power will be
assigned to carry out the work.

2.3.2.6 Concrete mix


a) Concrete Mix Design

Mix design will be prepared at site and given for approval, and then concrete cube
samples will be taken for each class of concrete.

Slump test will be conducted during mixing of concretes and the slump should be
within limits given or supplied by the engineer.

Compressive strength test to the conducted at 7, 14, or 28 days using compressive


strength machine in the presence of the engineer/ client and contractor.

b) Concrete mix

All concrete will be mixing by mixers

c) Transport of concretes.

One cone consistency (slump) test will be carried out when a test sample for
compressive and or tensile strength is made.

d) Pouring of the concrete

We will prepare concreting program and submit to the engineer before


starting any concrete activities.
Pouring of concrete will be carried out by concrete shooter and vibrator.
All reinforced concrete will be internally and externally vibrated.
Temporary curing seal will be supplied and applied for to free concrete
surface at the beginning of setting.

2.3.3 STONE MASONRY WORK

 Stone masonry will be packed cement mortared stone walls as indicated on


drawings constructed in a specified sand cement mortal having
compressive strength as indicated in relevant specification. Stone used will
be sound, durable, quarried. Weep holes will be provided as prescribed
and will be cleaned of mortal or any other clogging material that may have
entered during construction. They will be provided at the lowest point
where free outlets can be obtained and will be not more than 3 meter center

to center. Exposed parts of the wall faces will be cleaned of all mortal by
washing or wire brushing so that appearance of the completed wall will
present an even, tight surface.

 All masonry will be constructed by experienced workman. Face stones will


be set in random bond and care will be taken to prevent the bunching of
small stones or stones of the same size. The rolling or turning of stones on
the walls will be avoided.

At the initial stage of the project, the quarries with acceptable quality of rock for
masonry work and aggregate production will be selected and approved by the
Engineer. The exploitation of quarries shall be commenced as early as possible.

2.4 QUALITY ASSURANCE /MANAGERIAL & CONTROL


PROCEDURES
When the specification requires a contractor quality control program, the
contractor shall establish, provide, and maintain an effective quality control
program that details the methods required by this contract conform to contract
plans, technical specifications and other requirements, whether manufactured by
the contractor, or procured from subcontractors or vendors. Although guidelines
are established and certain minimum requirements are specified here in and
elsewhere in the contract technical specifications, the contractor shall assume full
responsibility for accomplishing the stated purpose.
a. Adequately provide for the production of acceptable quality materials.
b. Provide sufficient information to assure both the contractor and the engineer
that the specification requirements can be met.
c. Allow the contractor as much latitude as possible to develop his or her own
standard of control. The contractor shall be prepared to discuss and present, at
the preconstruction conference, his/her understanding of the quality control
requirements. The contractor shall not begin any construction or production of
materials to be incorporated into the completed work until the quality control
program has been reviewed by the engineer. No partial payment will be made
for materials subject to specific quality control requirements until the quality
control program has been reviewed.

The quality control requirements contained in this section and elsewhere in the
contract technical specifications are in addition to and separate from the

acceptance testing requirements. Acceptance testing requirements are the


responsibility of the engineer.
2.4.1 General description OF Quality Control Program
The contractor shall establish a quality control program to perform inspection and
testing of all items of work required by the technical specifications, including those
performed by subcontractors. This quality control program shall ensure
conformance to applicable specifications and plans with respect to materials,
workmanship, construction, finish, and functional performance. The quality
control program shall be effective for control of all construction work perfumed
under this contract and shall specifically include surveillance and tests required
by the technical specifications, in addition to other requirements of this section
and any other activities deemed necessary by the contractor to establish an
effective level of quality control.
2.4.2 Quality control program
The contractor shall describe the quality control program in a written document
that shall be reviewed by the engineer prior to the start of any production,
construction, or off-site fabrication. The written quality control program shall be
submitted to the engineer for review at least 14 calendar days before the
commencement.
The engineer should choose an adequate period for review. A minimum of 5 days
before the preconstruction conference or the start of work is recommended.
Submittal of the written quality control program prior to the preconstruction
conference will allow the engineer to review the contents and make suggestions at
the preconstruction meeting.

This will give the contractor a better understanding of the requirements before
developing the quality control program.
When selecting the required days for the contractor to submit the quality control
program, adequate time should be allowed for the quality control program to be a
supplement to the owner’s construction management plan.

The quality control program shall be organized to address, as a minimum, the


following items:
a. Quality control organization
b. Project progress schedule
c. Submittals schedule
d. Inspection requirements
e. Quality control testing plan
f. Documentation of quality control activities
g. Requirements for corrective action when quality control and /or acceptance
criteria are not met.
h. The contractor is encouraged to add any additional elements to the quality
control program that he/she deems necessary to adequately control all
production and/or construction processes required by this contract.

2.4.3 QUALITY CONTROL ORGANIZATION


The contractor quality control program shall be implemented by the establishment
of a separate quality control organization. An organizational chart shall be
developed to show all quality control personnel and how these personnel integrate
with other management/ production and construction functions and personnel.

The organizational chart shall identify all quality control staff by name and
function, and shall indicate the total staff required to implement all elements of the
quality control program, including inspection and testing for each item of work. If
necessary, different technicians can be used for specific inspection and testing
functions for different items of work. If an outside organization or independent
testing laboratory is used for implementation of all or part of the quality control
program, the personnel assigned shall be subject to the qualification requirements.

The organizational chart shall indicate which personnel are contractor employees
and which are provided by an outside organization.

2.4.3.1 Program administrator:

The program administrator shall be a full-time employee of the contractor, or a


consultant engaged by the contractor. The program administrator shall have a
minimum of 5 years of experience in construction of similar project and shall have
had prior quality control experience on a project of comparable size and scope as
the contract.

The program administrator shall have full authority to institute any and all actions
necessary for the successful implementation of the quality control program to
ensure compliance with the contract plans and technical specifications. The
program administrator shall report directly to a responsible officer of the
construction firm. The program administrator may supervise the quality control
program on more than one project provided that person can be at the job site
within 2 hours after being notified of a problem.

2.4.3.2 Quality control technicians


A sufficient number of quality control technicians necessary to adequately
implement the quality control program shall be provided. These personnel shall be
engineers, engineering technicians, or experienced craftsman with qualifications
in the appropriate field equivalent to NICET level II or higher construction
materials technician or Building construction technician and shall have a
minimum of 2 years of experience in their area of expertise.
The quality control technicians shall report directly to the program administrator
and shall perform the following functions:-
1. Inspection of all materials , construction, plant, and equipment for
conformance to the technical specifications, and as required
2. Performance of all quality control tests as required by the technical
specifications.

2.4.3.3 Inspection Requirements


Quality control inspection functions shall be organized to provide inspections for
all definable features of work, as detailed below. All inspections shall be
documented by the contractor as specified.
Inspections shall be performed daily to ensure continuing compliance with
contract requirements until completion of the particular feature of work. These
shall include the following minimum requirements.
a. During plant operation for material production, quality control test results
and periodic inspections shall be used to ensure the quality of aggregates
and other mix components, and to adjust and control mix proportioning to
meet the approved mix design and other requirements of the technical
specifications. All equipment used in proportioning and mixing shall be
inspected to ensure its proper operating condition. The quality control
program shall detail how these and other quality control functions will be
accomplished and used.
b. During field operations, quality control test results and periodic inspections
shall be used to ensure the quality of all materials and workmanship. All
equipment used in placing, finishing, and compacting shall be inspected to
ensure its proper operating condition and are within the plan dimensions, lines,
grades, and tolerances specified. The program shall document how these and
other quality control functions will be accomplished and used.

2.4.3.4 Quality Control Testing Plan


As a part of the overall quality control program, the contractor shall implement a
quality control testing plan, as required by the technical specifications. The testing
plan shall include the minimum tests and test frequencies required by each
technical specification item, as well as any additional quality control tests that the
contractor deems necessary to adequately control production and/or construction
processes.
The testing plan can be developed in a spreadsheet fashion and shall, as a
minimum, include the following.
a. Specification item number
b. Item description
c. Test type

d. Test standard
e. test frequency
f. responsibility
g. Control requirements.

The testing plan shall contain a statistically-based procedure of random sampling


for acquiring test samples in accordance with the standard specifications.. All
quality control test results shall be documented by the contractor as required.
2.4.3.5 Documentation
The contractor shall maintain current quality control records of all inspections
and tests performed. These records shall include factual evidence that the required
inspections or tests have been performed, including type and number of
inspections or tests involved; results of inspections or tests; nature of defects,
deviations, causes for rejection, etc; proposed remedial action; and corrective
actions taken.
These records must cover both conforming and defective or deficient features and
must include a statement that all supplies and materials incorporated in the work
are in full compliance with the terms of the contract. Legible copies of these
records shall be furnished to the engineer daily. The records shall cover all work
placed subsequent to the previously furnished records and shall be verified and
signed by the contractor’s program administrator. Specific contractor quality
control records required for the contract shall include, but are not necessarily
limited to, the following records;
2.4.3.6 Daily inspection reports
Each contractor quality control technician shall maintain a daily log of all
inspections performed for both contractor and subcontractor operations on a form
acceptable to the engineer.
These technician’s daily reports shall provide factual evidence that continuous
quality control inspections have been performed and shall, as a minimum, include
the following:
1. technical specification item number and description;
2. compliance with approved submittals;
3. proper storage of materials and equipment;
4. proper operation of all equipment;

5. adherence to plans and technical specifications;


6. review of quality control tests; and
7. Safety inspection.

The daily inspection reports shall identify inspections conducted, results of


inspections, location and nature of defects found, causes for rejection, and
remedial or corrective actions taken or proposed.
The daily inspection reports shall be signed by the responsible quality control
technician and the program administrator. The engineer shall be provided at least
one copy of each daily inspection report on the work day following the day of
record.
Daily test reports
The contractor shall be responsible for establishing a system that will record all
quality control test results. Daily test reports shall document the following
information:
1. technical specification item number and description
2. test designation
3. location
4. date of test
5. control requirements
6. test results
7. causes for rejection
8. recommended remedial actions
9. Retests.

Test results from each day’s work period shall be submitted to the engineer prior
to the start of the next day’s work period. When required by the technical
specifications, the contractor shall maintain statistical quality control charts. The
daily test reports shall be signed by the responsible quality control technical and
the program administrator.

2.5 Measures to ensure environmental safety

The site organization will have a safety officer and his helpers to monitor the day
to day working style and workmanship of the workers. Proper signs to protect the
workers will be put at the right position by this team. Advices will be also given to
each worker at all times to avoid any dangers to them or other workers. Proper
safety equipment will be provided to staff and the management and supervision
staff that will be used while on site. Any person visiting the site will also be
provided with the same service. A suitable room will be provided and maintained
for first aid/safety matters and will be removed after completion of work.
Temporary fences will be constructed around the site. Temporary guard house at
the entrance gate with reasonable size will be constructed and will be removed
after completion of work. Placement of equipment at their appropriate locations
and working condition will be controlled by the safety crew. Precautionary
measures will be taken to minimize noise and dust. We will take all possible
measures to prevent accidents during the execution of the works. It will also take
care of all works to be performed or already constructed, adjacent buildings or
roads nearby the site.

The safety organizer all together with the gang chief and the guard men will follow
up the movement of each worker with in the vicinity, in order to eliminate
unnecessary movements towards places under secure.

2.6 PROJECT EVALUATION

The whole project activities will be evaluated every week by the project
management staffs with going through the weekly report. This weekly report is
prepared in the project office by compiling and analyzing the collected data and
information’s of the activities performed.

The weekly meeting will always be led and handled directly by the project
manager. During his absence, it will be organized by the delegated person, mostly
project office engineer or construction engineer.

This weekly performance evaluation is done because it is a technique that helps to


provide a basis to find out tangible facts for decision making to those identified
problem areas.

2.7 WORK SCHEDULE

In accordance with the time frame (120 Calendar Days), the allocated contract
budget (3,939,437.77 ETB) and our method statement to cope up with the given
contractual obligation, we have prepared a preliminary work program considering
the detail on the contract documents (please refer to the work schedules).

Using the methodology of scheduling and method of resource scheduling inputs;


the master work schedule has been presented in bar charts to get the physical work
load distribution, financial cash flow, Equipment requirement and material
requirement for the whole contract period.
