AFFIDAVIT of UNDERTAKING

I, NARCISO L. HILARIO, of Legal Age, Filipino and a resident at 2008 Ipil St.
corner Herrera St. Sta Cruz Manila City, after having been duly sworn to in
accordance with the law, depose and say:

1. That I am the Registered Owner of Narcibel Power tools trading located at

554 Rizal Avenue, Sta, Cruz Manila City Barangay 307, A business enterprise
duly registered to engaged in the selling of Power tools and equipment;

2. That the NARCIBEL POWERTOOLS TRADING as registered in DTI and the

NARCIBEL ENTERPRISES as registered in Mayor’s Permit are the same

3. That this affidavit is being executed to attest to the truth of the foregoing facts,
to establish that NARCIBEL POWERTOOLS TRADING and NARCIBEL
ENTERPRISES refer to one and the same establishment for PAG-IBIG
Transactions.
