NARRATIVE REPORT ON THE IMPLEMENTATION OF ADOLESCENT HEALTH PROGRAM

Pursuant to the OUA MEMO 00-1020-0198 Memorandum entitled “Advisory on the
Implementation of the Medical – Nursing Services and Adolescent Reproductive Health Program for S.Y.
2020 – 2021, the Implementation of Adolescent Reproductive Health Program shall be community based
due to the current Pandemic COVID – 19 and be conducted by the Department of Health (DOH).

City of Malolos Senior High School joined the said program. As School Health Coordinator of our
school, all adviser of the school were coordinated to me to provide the list of their female learners.
Instead of going to the respective Rural Health Unit, I went to Barangay Health Unit of Guinhawa to
coordinate to their health unit worker if I can request to get the Folic Acid as per number of female
learners in the school. As a result, they granted our request and provide the number of Folic Acid as
requested.
