001, under Entry No. C- 01234, dated January 07,2020, the following entries of events exists to wit:

STABBING INCIDENT
On or about 8:00 in the morning of January 07,2020 concerned citizen reportee
Informed that there was a stabbing incident transpired along Cabanatuan City, Mobile Patrol
Car No 606 has been directed thru radio call to proceed to the scene of the crime,to verify the
reported, incident.

In the initial investigation conducted by the responding police officer disclosed that the
Victim
Indentified as Jose Santos y Medina,male married,22 years old,market vendor and presently at
Brgy.sta. Arcadia, Cabanatuan City, and the suspect indentified as Carlos Santos y Gomez, male,
Married, 25 years old, tricycle driver, and presently residing at Brgy. Magsaysay norte
Cabanatuan City. According to the suspect, Mr, Carlos G Santos stated that he stabbed his
cousin because that there was an misunderstanding between the report and the victim

DISPOSITION:
Patrol Car No. 606 has been directed thru radio call to proceed to the scene of the crime,
purposely to verify the veracity of the reported incident, and upon confirmation, the case was assigned
to the duty investigator on the said date.

 This certification is issued upon the request subject reportee of for whatever legal
purpose and intent it may serve.
