Project Goal and Envisaged Outcomes

The objective of enhancing the capacity of farmers in water management technologies is to intensify
in a sustainable manner agricultural production through improved water control by farmers, with
the envisioning outcome to reduce in gully soil erosion and to improve food security, increase farm
income and raise livelihoods in particular for the small-holder farmers in Nugaal and Bari regions of
Puntland state of Somalia.

Background and Rationale

Among increasingly scarce resources, water is becoming a constraining factor in agricultural


production and in sustaining life in urban and rural areas. Food security is inextricable linked to
water security and can only be achieved through a concerted action to raise water productivity,
maximizing crop production in both rain fed and irrigated farms. The advances from more
effective water management are considerable. A range of techniques and technologies are
available that will ensure a more effective use of water for agricultural production, both in rainfall
and in irrigated farms. Investments are required, but more importantly: advise, technical
guidance and financial support to the users to make sure that the initial constraints linked to the

introduction and management of new water management techniques and technologies are
adequately addressed.

Putting farmers in charge of Irrigation Management Systems along the technologies and techniques
has proved effective to achieve efficient and sustainable water management systems and to
increase water productivity. This is evident in the transfer of management of irrigation systems to
the immediate beneficiaries.
Explanation
Farmers’ Water Management Technologies and Techniques is the process in which individual
farmers and farmer institutions set objectives for the management of their water resources;
establish appropriate conditions, and identify, mobilize and use resources, so as to attain these
objectives.

This description of Farmers’ Water Management Technologies and Techniques applies to all types
of water-control systems, including irrigation, drainage and flood control systems and associated
activities that resort under the responsibility of farmers, either as individuals or cooperating.

Modern Medical Foundation (MMF) is a non-profit making Foundation that serves the needs of
vulnerable people in the Horn of Africa. MMF was founded on early 2020 by a group of
professionals and other community activists who are concern the wellbeing of Somali people

who have been affected the conflict and other natural disasters including drought, flooding,
Malaria, Pandemic etc. MMF focuses on supporting and improving the poor, the oppressed and
vulnerable communities, especially women and children. (MMF) is committed to supporting
sustainable development heath programs through organizing local community groups such as
women and youth at the grassroots levels and supporting them.

MMF owns and runs Modern Hospital in Galkaio, Somalia. Modern Hospital is one of the main
referral hospitals in Galkaio that provides a range of free or subsidized medical services to a
large population of patients from central Somalia and eastern regions of Ethiopia. The hospitals
provides health care to outpatients, inpatients, postnatal visits, deliveries and emergencies. The
hospital has dedicated and specialized departments including general medicine, genecology,
various branches of surgery, paediatrics, obstetrics, psychiatric and modern laboratories. All
manned by both national and international medical specialists.

Thereby, we are dedicated and privileged in achieving result oriented outcomes to every
interventions in hand and meeting stakeholders’ needs.

Methodology and approach

In order to successfully implement this project, it’s planned to carry out some tasks and activities to
address and achieve the desired outcomes for the persons of concern.

MMF will commence registration of the beneficiaries who have a problem of cleft lip and palate

accordingly. The type of the operation would be determined by the surgeon and his team, then the
patient will be screened, then Anastasia will be carried out, the operation will be started and would take
1 hour and half for the simple operations while the complicated cleft lip and palate ENT would take 2.5
hour according to the situation of the patient. The patient will be hospitalized around two to three days
after that the patient will visit the hospital for two weeks for follow-up and treatment.

MMF will assign a technical team to monitor the effectiveness and efficiency of the operations in order
to make sure that the project is following strictly under the guidelines and objectives set for the project.
This project will be implemented by the Modern Hospital in Kalkio-Puntland who have been carrying
out such operations since the last 5 years and is currently operate three to four cases in every day. This
operation will be carried out a team consist of 10 people

Resources

In aligning with financial policy of the MMF foundation, the resource would be mobilized in the
following ways in order to successfully done this project. We planned to collect 70% of the
total project cost from the local Banks, charity institutions and other goodwill people who are
concerning about the health interventions of their community while, the remaining 30% will
be contributed by the Modern Medical Foundations.

Technical capacity

Modern Hospital has wide-ranging experience and strong reputation in ENT surgery, Cleft and
Palate operations, Health awareness, supporting vulnerable people in the community. The
hospital carry out 3-4 cases of cleft and palate operations successfully in every day.

These ENT operations are led by the following Technical Surgeon Doctors mentioned in the
below:-

1. Dr.Amir Khalaf Otolaryngologist: Master in ENT surgery and expert in ear, nose and
throat surgical procedures. Otolaryngeal plastic surgeon which included cleft lip and
palate reconstruction. He has done more than 1000 cleft lip and palate operations
successfully.

2. Dr.Abdirahaman Al azhari, PhD in surgery, member of Royal College of Surgeons. Expert


in general surgical operations, lipomas, appendectomy, hernia and dealing traumatic
emergency with 8 year of experiences.

3. Mohamed Jama Ali, pediatric surgery with 11 years of experiences.

deliver to deliverables and services so as that, Modern Hospital requires a support in
improving some of the following areas.
A. Expending and renovating Hospital building
B. Ophthalmology and Neuroethology.
C. MRI and CT-Scan

Expectations

Modern Medical Foundation is expecting and confident that their proposed concept note
will be impactful for the community living in Mudug region-Puntland, Somalia.

MMF is looking forward to support the vulnerable people in our community and also to
meet the expectations of the donors for this project. Meanwhile, Setting up best and
reasonable initiative that will improve outcome mechanisms and facilitate the reporting
and follow-up for the beneficiaries.

This proposed idea is expecting to be implemented with a total budget of 100,000 USD,
thereby, We are requesting to have a fund with this project of a total amount of 70,000
USD while, MMF is hereby guaranteeing to make a contribution of 30,000 USD and some in-
kind contributions like, free follow up for six months after the operation has been
completed.

the ENT and other cross-cutting health issues thereby, we are assuring that our initiative is
