CONTRACT OF SERVICE
KNOW ALL MEN BY THESE PRESENTS:

This contract of Service is made and executed by and between:

OLD SOMOROY ELEMENTARY SCHOOL, a government entity with office
address at Brgy. Somoroy, Lope de Vega, Northern Samar, represented by its School-IN-
Charge MS. SALVACION A. GONZAGA, hereinafter called the FIRST PARTY;
-and-
JEFFREY BASILOY., Filipino citizen, of legal age, with address at Brgy
Somoroy, Lope de Vega, Northern Samar, hereinafter called the SECOND PARTY;
WITNESSETH:

1. That for and in consideration of the sum of Five Thousand Five Hundred Eighty-Six Pesos (P
5,586.00), Philippine currency, the SECOND PARTY shall perform as carpenter for the school
Fence (pakyaw) and supplier of all the wood materials of the said repair in Old Somoroy
Elementary School on June 17, 2019.

2. That the FIRST PARTY shall pay the SECOND PARTY the sum of Five Thousand Five Hundred
Eighty-Six Pesos (P5,586.00), Philippine currency.

IN WITNESS WHEREOF, we have hereunto set our hands this 2nd day of June, 2019
at Brgy. Somoroy, Lope de Vega, Northern Samar, Philippines.
