AFFIDAVIT

I, ____________, of legal age, single, Filipino, with residence and

postal address at 36 IB Upper Calarian, Zamboanga City under oath do
hereby depose and say:

1. That I am the daughter of _____________ who has passed away on

________________;

2. That the said deceased acquired a loan with KWIKLOAN Lending

Corporation prior to his death but the undersigned affiant has
made full payment of the said loan with the company;

3. That by virtue of the loan, the deceased’s automatic teller

machine (ATM) card was left in the possession of KWIKLOAN
Lending Corporation;

4. That I am executing this affidavit to attest to the veracity of the

foregoing facts, to claim the deceased’s ATM card with the
KWIKLOAN Lending Corporation and for other legal purposes it
may serve.
