Overview of the Assessment Activity (Provide a clear and concise description of your

e-Portfolio/Portfolio
The learners are tasked to make an e-portfolio (PPT) /portfolio containing disaster
preparedness practices of their community (preferably their barangay) in case of
earthquakes, tsunamis, and volcanic eruptions. The learners should also state his/her
suggestions for the improvement of the existing community plans.
Consider the following guidelines and instructions:

Make an essay about the disaster preparedness of their community and his/her
suggestions to further improve the plans of the community. Use persuasive words to
convince your audience.
MATHEMATICS
Show the disaster preparedness for earthquakes by attaching the pictures to your
chosen shapes using a geometric sequence. (Before, During, and After Scenario)

SCIENCE
Make a survey or research about the disaster preparedness plan of your community
(barangay) to mitigate the effects of natural calamities such as earthquakes, tsunamis,
and volcanic eruptions (you can attach actual related photos). On a separate slide,
discuss your suggestions for your community and for other government organizations
for earthquake and other disaster mitigation as the contents of your essays and other
slides.
MICROBIOLOGY
Make an informative essay about 3 microbial diseases that could spread in the
community after natural disasters such as earthquakes, tsunamis, and volcanic
eruptions. Discuss their causative agents, how they can spread in the community, and
suggest disaster preparedness plans to control these diseases.
