Creating a QUIZ or EXAM in Google Forms

STEP 1: Add materials by selecting the ‘Create’ button.

STEP 2: Select ‘Quiz assignment’ from options.

STEP 3: A ‘Quiz assignment’ material will automatically generate a ‘blank quiz’ through Google
Forms. Select ‘Blank Quiz’. Then proceed to edit/type your quiz items.
STEP 4: Select ‘Answer key’ on the lower left corner. Then set the correct answer. The form
will automatically check the quiz.

Select the correct answer, and click ‘Done’ on the lower right corner of the form.
Choice ‘B’ is set as the correct answer.

STEP 5: Repeat STEP 4 when adding more items.

STEP 6: (1) Make sure that the form is set as a quiz. (2) You can also set what students can see
such as ‘missed questions’/items, ‘correct answers’ and ‘point values’ for each item/question. If
(3) you want students to view their scores, on the ‘Release grade’ menu, choose whether to
release immediately or later.

STEP 7: PRIVACY SETTINGS: (1) Check ‘Restrict to users in Our Lady of the Pillar College-
Cauayan and its trusted organizations’ if you want students to answer using their olpcc.edu.ph
Gmail account. If some students do not own an olpcc.edu.ph Gmail account, then uncheck it.

(2) Make sure that ‘See summary charts and text response’ option is unchecked. This option, if
checked will allow all students to see responses of their schoolmates.

PREVIEW AND RESPONSES

By selecting the preview button, the instructor can see how the Google form quiz will appear to
Students.
Select any of the choices. Since it was set that students can view score immediately. They can
look at their scores by selecting the ‘view scores’ button after they submit the form/exam.

The correct answer is ‘B’.


Close the form. Make sure it is saved.

STEP 8: Refresh the page (in Google Classroom) on which you created the ‘Quiz’/Exam. The
Form is now saved. You can copy the ‘Quiz’ to other classes: just check which class you want to
copy the form/’Quiz’ as a material. You can set the deadline manually, using the side
pane/menus.
PUBLISHING/ASSIGNING

STEP 9: Select ‘Assign’ to publish the form. The students can view it under ‘Classwork’ in Google
Classroom.

STEP 10: Scheduling a Google Form: Click the dropdown arrow beside the ‘Assign’ button.
Select ‘Schedule’, then set the desired time and date when the Form will be open/available to
students.

STEP 11: Make sure that the created material (Form or Quiz) appears in ‘Classwork’.
