Instructions for students:

1. Write your name above the table.

2. Write the name of team member in the table.

3. The three criteria of how you assess your team member are: -

• COOPERATION: Overall, how effectively did your team member work together on this project?
• PARTICIPATION: Overall, how much contribution did your team member give to the project?
• ATTITUDE: Overall, did your team member show a positive or negative approach in working towards your group’s goal?

4. Write down the rating scale points (number) for each team member based on the three criteria (Cooperation, Participation and
Attitude) using the 5-point rating scale given below:

5 – Outstanding
4 – Very satisfactory
3 – Satisfactory
2 – Fairly satisfactory
1 – Poor

5. You MUST write at least one sentence in the feedback section for each team member. Your comment may be negative, positive
or even neutral. If left blank, your form will not be accepted and assessment will not be considered.

6. Please be HONEST and UNBIASED when assessing your team member. This form will be kept CONFIDENTIAL.

7. After completing the form in Microsoft Word, save it as pdf and upload on LMS or email it to yusrina.yakub@pb.edu.bn.
