Dear Sir,

Consulting India Pvt. Ltd (‘Company’).

2. As a part of defined exit formalities I am liable to surrender all Company


provided assets and documents provided to me by the Company.

3. I hereby declare that I have the following Company provided assets and
documents under my possession at present.

4. I am not able to surrender the above mentioned Company provided assets


and documents due to the current lockdown situation.

6. I acknowledge that I will be solely responsible in case of any unauthorized or
illegal use of the Company provided assets, till the day these assets are
surrendered back to the Company and such surrender acknowledged in
writing by the authorized representative of the Company.

7. I hereby agree to abide by the ISMS policy of the Company in reference to the
possession of the Company provided assets and documents. I completely
understand that any breach of ISMS policy may cause a financial loss or
reputation loss to the Company and the Company can take any action against
me as desired by the Company.

8. I hereby authorize the Company to recover all the costs, damages incurred
due to any unauthorized misuse, loss or negligence by me or on my behalf by
deducting any amounts due to me from the Company and by sending
demand notice to me for any balance amount.

9. I acknowledge that the Company may use all the legal remedies available
including but not limited to imposing fines, filing criminal complaints or legal
suits as deemed suitable by the Company for any acts considered to be a
breach of any policies or prevention of actions which may lead to breach of
the policies.
