1. This has reference to your reports relative to the Robbery incident transpired at the
residential house of one Alfredo Caldito y Ayala of Brgy. 10 San Agustin, Bacarra this province
on March 01,2021 at the aforementioned place wherein the suspects was Mark Ravelo y
Otanes, Francis Abad y Domingo, Rommel Vila y Eda and James Yap y Reyes.

2. In view of the foregoing, you are directed to submit to this Office, Attn: C, PIDMB,
your Investigation Report NLT May 10, 2021.

sabihin ang aking nalalaman at para narin mapanagot ang mga may sala a
naganap na nakawan.

S - Sa Brgy. 10 San Agustin Bacarra Ilocos Norte gabi ng Marso Uno,
Dalawang libo Dalawamput isa.

S - Sila po ay sina Mark Ravelo Otanes,Francis Abad,Domingo,Rommel Vila

S - Hindi kopo nasaksihan ng buo ang pangyayari pero nakita ko silang apat
na umaali-aligid sa bahay ng mga Ayala

8. T - Kilala mo ba itong biktima?

S - Opo sila po ay maimpluwensiya sa aming lugar at kilalang mayaman.

9. T - Sa ikaliliwanag ng pagsisiyasat na ito, maaari mo bang isalaysay o sabihin

kung ano nag tunay na nangyari sa naganap na nakawan?
S - Gabi na nuong nakita ko silang apat na nakasakay sa dalawang motor
pero diko nalang pinansin at hindi nireport sa Barangay sa kadahilanang
akala ko ay di sila gagawa ng krimen.

10. T - Sa iyong pagkakaalam, ilan sa mga suspek ang may mga dalang baril at
anu-anong mga kalibre nito?
S - Hindi po ako sigurado kung ilan sa kanila ang may dalang baril at kung
 anung klaseng baril ang meron sila.

S - Hindi kopo alam kung meron papo.

S - Wala po akong ginawa kasi po hindi kopo nasaksihan mismo ang
nangyaring nakawan.

13. T - Kung muli mo silang Makita at makaharap, makikilala mo ba ang mga

ito?
S - Opo makikilala kopo sila.

14. T - Pansamantala ay wala na akong itatanong sa iyo, mayroon ka bang

ibang ibig idagdag o bawasan sa salaysay mong ito?
S - Wala napo.

15. T - Ikaw ba ay tinakot, pinangakuan, o binigyan ng pabuya sa pagbibigay

AFFIDAVIT OF ARREST

We, P/Msgt. Roy S. Natividad and P/Msgt. Jerome C. Fernandez of legal age,
bonifide member of the Philippine National Police and presently assigned at Bacarra Municipal
Police Station, after having been duly sworn to in accordance with the law, do hereby depose
and state:

1. That on March 01,2021 our station received an information from Alfredo C. Ayala
of Brgy. 10 San Agustin Bacarra, Ilocos Norte that there was a robbery incident at the Ayala`s
residential house.

2. That, acting on thar information, our Commanding Officer directed us to respond

at the reported place of incident to verify the veracity of the said information and if affirmative, to
conduct an ocular investigation;

3. That, upon arrival at the place of incident, we were personally informed by the
complainant that the victim one Alfredo C. Ayala was robbed his residential house at about 8:00
PM; and that, we were likewise informed that the suspect escaped after the incident with the
personal belongings they carted away;

4. That, we immediately informed our station confirming this incident; and

thereafter, I and Police Master Sergeant Jerome C. Fernandez and Andres S. Mananambit who
witnessed the incident and volunteered to help us. Proceeded at the residence of the suspect at
Barangay 15 San Vicente Bacarra, Ilocos Norte and upon arrival thereat, our witness positively
identified the suspect who was then sleeping because of drunkenness at their barrack;

5. That, we immediately identified ourselves to them (Mark Ravelo Otanes. Francis

Abad Domingo, Rommel Vila Eda and James Yap Reyes) and arrested the same; and we
likewise informed them the reason of their arrest and of their constitutional rights; however,
during their arrest, the personal belongings they carted away was not recovered from their
possession;

6. That, them (Mark Ravelo Otanes. Francis Abad Domingo, Rommel Vila Eda and
James Yap Reyes) submitted theirself to us peacefully; and further identified theirself as one of
the suspect of robbery incident and that, we immediately brought them to our station for medical
examnation, investigation and proper disposition; and

7. That we executed this affidavit to attest the veracity of the aforementioned facts
and to support the filing of appropriate charge/s in court against the arrested suspects;
