ACTION REPORT
Dear Sir
The following report presents the results of the vibration survey for the shipboard machinery onboard CSC Dynamic
Vision at anchorage in Fujairah dated 5th January 2008. The objective of the survey is to assess the service condition of
surveyed equipment and diagnose those found to be unsatisfactory with corrective actions. The survey also enables
clients to satisfy requirements of Special Survey of Machinery in accordance with American Bureau of Shipping (ABS)
Guide for Survey based on Preventive Maintenance Techniques.
The survey was conducted between the 4th to 7th January 2008 and was assisted by the Chief Engineer of CSC Dynamic
Vision. Most equipment were surveyed and four (4) equipment were found to have bearing fault which have persisted
over the years. However, fault signature was found to be stable and equipment is deemed fit for continued service.
The entire report has been expressed and documented without prejudice. The arrival of the executive summary, results &
recommendations herein are solely limited to vibration analysis, and is based on related experiences, empirical
references and expertise during the course of our professional consultancy services.

2.0 Instrumentation
The following instrumentation were used to perform the vibration measurement:
Summary of Result : Equipment condition is satisfactory at time of survey. The vibration was
dominated by the operating frequency of the motor. Bearings condition
exhibited intermediate fault at motor and pump end bearings. Fault condition
has indicated signs of deterioration.
Summary of Result : Equipment condition is satisfactory at time of survey. The vibration was
dominated by the operating frequency of the motor. Bearings condition were
satisfactory at time of survey with the exception of the pump drive end which
exhibited intermediate fault.
Summary of Result : Equipment condition is satisfactory at time of survey. The vibration was
dominated by the operating frequency of the motor. Bearings condition show
signs of deterioration as compared to the last survey.
Summary of Result : Equipment condition is satisfactory at time of survey. The vibration was
dominated by the operating frequency of the motor. Bearings condition show
signs of deterioration as compared to the last survey, similar to that of Stern-
tube LO Pump #1.
