OFFICE POLICIES AGREEMENT

DURATION OF SESSIONS: Individual sessions are scheduled for 45 to 50 minutes. Group and assessment sessions
will be discussed at the beginning of intervention.

PAYMENT: Clients are responsible for all charges incurred and services must be paid in full at the time of the visit
unless other written arrangements have been made in advance.

Please be aware that some of the services offered by a Licensed Educational Psychologist can be obtained through
the local school district for free.

INSURANCE: At this time, Dr. Akridge is not an in-network provider on any insurance plan. If you wish to submit the
invoice to your insurance company, the invoice provided for services contains all information needed to submit your
claim so you can pursue reimbursement for services as defined under your plan/policy. There is no guarantee that
you will be able to receive reimbursement for services and it is each client’s responsibility to consult your insurer for
benefits related to Dr. Akridge’s services. If need be, Dr. Akridge can provide an estimated cost invoice with
appropriate billing information to be able to discuss with your insurance plan prior to services beginning.

CANCELLATIONS: The scheduling of an appointment involves the reservation of time specifically for Dr.
Akridge and the client. To avoid being charged for a missed session, a cancellation notice of at least 24 hours must
be given. Cancellations can be made by calling or emailing Dr. Akridge. A late cancellation will result in a forfeit of
your deposit in the event that your all cancellations made less than 24 hour before the appointment.

EMERGENCY PROCEDURES: Dr. Akridge can be reached at (877) 619-6701 and an attempt will be made to return
all calls within a timely manner. Please keep in mind that if a message is left late in the day, a return phone call may
not occur until the following day. As client’s phone numbers are not always accessible, please be sure to leave your
phone number during a message. If you are experiencing a life or death emergency, please phone 911 immediately.

RECORDS: Records are maintained of the intervention/assessment for a period of at least three (3) years after the
completion of intervention/assessment. Reports will be provided to the client after an assessment is completed within
5days of the conclusion of the evaluation.

OTHER INFO: On occasion clients often run into their educational psychologists in public places outside of the office
(stores, markets, etc.). To maintain your confidentiality, it is your choice to say hello; that decision will be left up to
you.
