MEMORANDUM OF AGREEMENT (MOA)

KNOWN ALL MEN BY THESE PRESENTS:

This Memorandum of Agreement (MOA) made and entered into between;

NDC- TAGUM FOUNDATION, INC. represented herein by the LUDINA M.

OBALLO, LPT, Ed.D., Academic Dean & Program Head, College of
Education hereinafter referred to as "FIRST PARTY"

and

PANGI NATIONAL HIGH SCHOOL, a secondary public school in Maco

North District with School ID, 301526 situated at Purok Mangosteen,
Pangi, Maco, Davao de Oro represented by the School Principal I, EDNA
G. OMBECAN herein referred to as “SECOND PARTY”.

WITNESSETH:

WHEREAS, Bachelor of Secondary Education (BSEd) students of NDC-

Tagum Foundation, Inc., Apokon, Tagum City will undergo Field Study 1 and Field
Study 2 in Pangi National High School assigned in their respective cooperating
teachers;

WHEREAS, Field Study students of NDC-Tagum Foundation Inc. wish to

contract for benchmarking in Pangi National High School to gain experiences in the
actual environment as part of the requirements of the curriculum they are undertaking;

WHEREAS, the new normal policies and guidelines on the deployment of

pre-service teachers for field study and teaching internship for AY General was
adopted and strictly followed by the NDC Tagum Foundation, Inc.

WHEREAS, it is stressed that for teaching internship, it is the responsibility of

the assigned college supervisor to facilitate the activities, monitor, and evaluate the
performance of the PTs.

WHEREAS, Pangi National High School shall accommodate observations

and interviews if needed;

WHEREAS, cooperating teachers will act as coaches and mentors of the PTs
in their teaching internship program as Pangi National High School, the partner or
laboratory school;

NOW, THEREFORE, for and in consideration of the foregoing, the PARTIES

do hereby agree to undertake its respective commitments under the following terms
and conditions, viz-a-viz;

ARTICLE I- COMMITMENT AND COOPERATION

The roles and responsibilities of each party are defined and enumerated in this article
as follows:
SEC 1. NDC-TAGUM FOUNDATION, INC. shall:
 a. Field Study Students (FSS) will observe the teaching-learning process in
Flexible Learning Delivery Mode (printed module modality) of Pangi National
High School and its on-site activities;

b. Assist the Cooperating Teacher in the preparation and implementation of

printed module modality classes;

c. Assist the CTS in preparing class activities;

d. Assist the CTs to create assessment materials related to the lessons,

applicable to the modality that Pangi NHS adopted;

e. Assist the cooperating teachers in checking students’ outputs;

f. Assist the CTs in accomplishing school forms;

g. Assist the CTs in parent-teacher conferences;

h. participates in faculty meeting, auxiliary tasks assigned and join

professional seminars

SEC. 2. PANGI NATIONAL HIGH SCHOOL as an existing development partner

shall continue to:
a. Welcome and accommodate the FS students from NDC-Tagum
Foundation, Inc;

b. As laboratory and partner school, Pangi NHS will provide cooperating

teachers who will guide, lead and facilitate the field experiences of our pre-
service teachers;

c. In order to promote the well-being of students and guarantee quality of their

learning and exposure and ensure their safety while undergoing internship,
Pangi National High School as cooperating school will adhere to the
requirements on Student Internship Program;

d. Cooperating teachers will see to it that the tasks assigned to the Field
Study Students are consistent with the objectives of the Field Study and
limited to the specific requirements and activities of the courses;

e. Provide technical assistance to the FS students such as sharing vital ideas,

providing seminar and workshop which will enrich their understanding and
knowledge about pedagogical schemes.

ARTICLE II- AMENDMENTS AND MODIFICATIONS

The herein cooperating parties may amend, modify, or supplement this agreement upon
mutual consent. Amendments will be effective immediately once signed by all parties.

IN WITNESS WHEREOF, we have hereunto affixed our signatures in behalf of our respective
organization this 10th day of November 2021 during the 1-Day Training-Workshop on
School Forms & Lesson Planning at Purok Mangosteen, Pangi, Maco, Davao de Oro,
Philippines.
