last two (2) years prior to the deadline for the submission and
4 receipt of bids.

List of Completed Government and Private Contracts in the last

ten (10) years and Statement of Single Largest Completed Contract
5 (SLCC) (as stated in the Invitation To Bid).

6 Philippine Contractor's Accreditation Board (PCAB) License.

Audited Financial Statements, stamped "Received" by the Bureau
of Internal Revenue (BIR) or its duly accredited and authorized
7 institutions for the preceding calendar year, which should not be
earlier than two (2) years from bid submission.

8 Duly signed Computation of Net Financial Contracting Capacity

(NFCC) and/or Credit Line Commitment (CLC).
9 If applicable, the JVA in case the joint venture is already in
existence, or duly notarized statements from all the potential joint
venture partners stating that they will enter into and abide by the
provisions of the JVA in the instance that the bid is successful.

B. Technical Documents
1 The Bid Security shall be in the form of a bid securing declaration,
or any of the following forms and amounts:
a. 2% of the ABC, if bid security is in cash, cashier's/managers
check, bank draft/guarantee or irrevocable letter of credit.

b.5% of the ABC, if bid security is in surety bond.

2 Other Documents/Project and Contract Requirements:

a. Organizational Chart.

b. List of Contractor’s Personnel.

c. List of Contractor’s Equipment Units.
