Accreditation Checklist – NEW Application
Important Notes

▪ In order to achieve savings in cost and time, Agent is requested to submit their Application electronically
via Customer portal only.
▪ For the files to be uploaded to IATA portal, please use only pdf format.
▪ Please upload one file per document type. Uploading of multiple files for the same document type may
lead to rejection of your application.
▪ Please prepare in advance the following documents for submission. Incomplete submission of the
document may lead to a rejection of application
▪ Once you are ready, please fill up the online Application form and submit the signed form to IATA.
▪ During the application process we may request additional documentation.
▪ Please download application related fees here.
▪ All legal documents must be accompanied by an English translation by a certified translator.

HEAD OFFICE (HO)

Document Required Checklist
Application Form ●
Log in to the IATA Customer Portal to fill in your Application Form
online. Once completed, please download and sign the form to be
submitted as attachment to the case. The Form should be duly signed
by authorized signatory and affixed with company common seal (if
applicable).

Legal Document ●
If business entity: Company
• Notary Act of the company establishment and if applicable the latest
notary act on changes
• National Tax Number certificate (NPWP)
• Please kindly provide the following document :
o Valid Surat Izin Usaha Jasa Pengurusan Transportasi issued by
Dinas Perhubungan for freight forwarder company ; or
o Valid Surat Izin Penyelenggaraan Jasa Titipan issued by
Menkominfo for courier company
• All owners passport / identity card, if the shareholders are
corporation / company, please provide the certificate of
incorporation of the parent company
• Valid Tanda Daftar Perusahaan (TDP)

Basic/ Advanced Cargo Certificates ●

Two (2) Basic/ Advanced Cargo Certificates or equivalent Cargo
Course Certificates

 Dangerous Goods Training Course Certificates ●
Two (2) Dangerous Goods Training Course Certificates from any IATA,
Airline, IATA/FIATA or IATA DGR Authorized Training School (DGR
certificates submitted must not exceed (2) two years from the date of
issue)

Letterhead ●
Sample of Company Letterhead

Financial Statement ●
Latest certified and audited financial statements (Age of account must
not be more than six (6) months old at the time of submission)
The Financial Statement must include following:
• Auditor/Director's report
• Balance Sheet
• Profit & Loss Account
• Schedules

Office Lease or Rental Agreement ●

Office Lease or Rental Agreement or Documentary evidence of
ownership

Insurance Policies ●
Insurance Policies to cover agents’ liability for loss of or damage to
shipper’s cargo from Fire, Theft/ Burglary, Public Liability or Bailee

Financial Security ●
The level and mandatory sample text will be informed after receipt of
the financial statements

English Translation ●
If document is in foreign language, a translation in English is required.
All translation documents must be done by certified translator, high
commission/embassy or notary public.

BRANCH OFFICE (BR)

Legal Document ●
If business entity: Company
• Notary Act of the branch establishment
• Valid Domicile letter

Basic/ Advanced Cargo Certificates ●

Two (2) Basic/ Advanced Cargo Certificates or equivalent Cargo Course Certificates

One (1) Dangerous Goods Training Course Certificate from any IATA, Airline, IATA/FIATA or IATA

Office Lease or Rental Agreement or Documentary evidence of ownership

Insurance Policies ●
Insurance Policies to cover agents’ liability for loss of or damage to shipper’s cargo from Fire,
Theft/ Burglary, Public Liability or Bailee
