INSTRUCTIONS FOR OLFU VACCINATION RECIPIENTS:

 BEFORE VACCINATION:
1. Have your Antipolo Bantay Covid QR Code ready to show to the guard.
2. Bring your own ballpen and any valid ID.
3. Wear your mask, face shield and bring your hygiene kit (alcohol or sanitizer, tissue paper).
4. The vaccination team strongly advise you to be punctual and conscientious with your pre-
determined time to avoid long lines and to facilitate the proper timing of the vaccine
administration.
5. To pre-register to the Antipolo Bantay COVID Vaccination, follow these simple steps below:
a) Go to https://antipolobantaycovid.appcase.net/
b) Click on Vaccination Registration icon (color red on the screen)
c) Log in using your Antipolo Bantay COVID User Name and Password
d) Fill out the following IMPORTANT FIELDS (kindly indicate OLFU Antipolo - KM 23 Brgy Sta.
Cruz, Sumulong Highway, Antipolo City address as your address)
e) Valid ID (Click from the selection and you will need to upload the ID using your camera).
f) Wait until uploading is completed.
g) Check the “tick box” for the consent and then hit the “Submit” button.
6. Get enough sleep.

VACCINATION DAY:

1. Eat breakfast. Don’t go to the vaccination site with an empty stomach. Bring water.
2. Wear proper attire – shorts and t-shirt allowed except flip flops, slippers, and the like.
3. DO NOT DRIVE GOING TO THE VACCINATION SITE.
4. Be punctual and arrive at the vaccination site (OLFU Antipolo) 30 mins before your schedule.
5. Patiently wait for your turn if the lines are long. We have assigned marshalls and staff who will
attend to vaccination recipients.
6. Have your documents on hand during screening, i.e., list of maintenance meds, if any and
dosage; test results as may be required to check your medical history.
7. You will not be allowed to go home right after your vaccination as the medical team will observe
your condition accordingly. Just stay at the holding area/post-vaccination area.
8. Refrain from engaging in loud conversations nor loitering around the vaccination site. Just
remain on your designated seats/area.
9. Report to the vaccination team any adverse feelings you may experience after being vaccinated.
10. DO NOT DRIVE GOING HOME.

AT HOME:

1. As much as possible please refrain from posting your vaccination pictures in any social media
platforms.
2. Be very discreet in sharing your vaccination experience to your friends and family members.
3. Rest, take plenty of fluids and follow instructions to do at home as given by the vaccination
team.
