Job Title Care Planner and Coach


Team Carer Support Services

Reporting to Team Manager – Carer Support Services


Hours of Work Full Time (38 hours per week)
Employment Term Fixed Term contract until 30 June 2022 (with likely extension in line with Government
Funding)
Classification Carers ACT Enterprise Agreement 2014 SACS Level 4

Salary Range $68500 - $73700 per year + superannuation (remuneration dependant on experience and
qualification)
Location This role is located primarily at Holt. The position will require some travel time to other sites
within ACT.
Position Overview The Care Planner is responsible for the provision of high-quality client services to support and
develop the capacity of carers living in the ACT. The Care Planner will assess carer’s needs,
develop support plans and budgets and coordinate support services for carers. The position
liaises with brokerage services and other service providers. From February 2021, the planner
will also provide coaching to carers under the Carer Gateway Coaching model.

A carer is someone who provides daily care to someone in their family, or a friend, who has
chronic ill health, is disabled, frail aged, has a mental health condition or requires palliative
care and living at home (i.e. not in Aged Care Facility or Disability Supported Independent
Living).

To be eligible for support from Carers ACT a carer will be the main (primary) carer and spend
a considerable amount of time assisting the cared for person with the activities of daily
living. There are also specific eligibility requirements for clients to receive support under
various government funding contract.

The approach to working with carers is to support their personal wellbeing and capacity
building.

Key responsibilities Needs Assessment and support planning


Carer support, and The Intake officer will complete the carer intake and registration process and refer the
delivery carer to the Care Planner who will
• Using the Carers Star™ (Carers Star™training will be provided) undertake a
comprehensive and holistic needs assessment considering all relevant aspects of the
carer’s circumstances
• Identify what types of supports/services will benefit the carer, in line with their
assessed needs
• Identify relevant support and services
• Develop an Action plan to document the carer’s goals and the actions that will be
undertaken to support them

• Complete the Action Plan by recording the services the carer has chosen to use and
action any referral
• Planning supports for a carer must accommodate needs and preferences for service
delivery that are specific to their cohort (including but not limited to Aboriginal and
Torres Strait Islander, Culturally and Linguistically Diverse (CALD) and Lesbian, Gay,
Transgender, Intersex and Queer (LGBTIQ) people).
• Where the carer is eligible for financial support package, develop a budget and
expenditure plan in accordance with guidelines.

Coordination
• Coordinate and arrange of services through referral to appropriate providers
• Arrange brokerage of services on the carer’s behalf
• Provide information and immediate practical steps to enable carers to self- refer
• Support delivery of services to the carer by appropriate service providers

Monitoring
• Check -in with Carer and monitor progress against the action plan
• As part of follow up, complete a follow up assessment using Carers Star™ and
review against the baseline Carers Star™
• Where goals are met, exit carer from service or update the Action Plan and help
carers if additional relevant services are required

Coaching (from February 2021)

• A carer’s needs assessment and action plan may identify that the carer will benefit
from coaching.
• Care planner coaches will work with carers to identify their goals and the skills that
will be developed using a core set of skills that have been identified as critical to the
caring role
• The Care planner coach will conduct coaching sessions over a defined period
following the coaching program (training will be provided).
• Coaching sessions will be delivered in-person and/or by telephone) will be
specifically designed to assist carers to acquire the skills and resilience needed in
their caring role

Carers ACT will have a small team. Care Planner Coaches will be cross trained and required
to complete the functions of the Intake Officer from time to time:
Intake & registration:
• Respond and Engage with Carers calling on Carer Gateway national 1800 number
• Determine in the initial call what the carer requires
• Assess urgency and coordinate the delivery of services accordingly
• Arrange Emergency respite care, as a matter of urgency

• Provide information and referral to other services including Carer Gateway online
services
• Determine eligibility to access carer support services
• Educate the Carer about the Carer Gateway Services and Carers ACT’s Services and
refer to the most suitable service
• For eligible carers collect and document Carer and Care recipient personal
information that is most useful to support service delivery and provide an efficient
carer experience
Service Development • Identify systemic advocacy issues and service gaps and make recommendations for
additional services where appropriate
• Propose recommendations for continuous improvement
• Contribute to ensuring that program outcomes and outputs, budgets and financial
accountability requirements are met
Community • Represent and promote Carers ACT at meetings, networking and other community
Development development events.
Media and • Contribute to organisational marketing and promotional activities including
Communication contributing to newsletters and representing the organisation at community events
Data Collection and • Maintain and keep up-to-date Carer and their care recipient personal records in

• Update Carer Action Plan if additional services are required
• Record Carer Wellbeing and circumstances before and after services in the
recommended format and Carers ACT CRM system
• Review, update, or revise file contents to reflect current status of registered Carers
• Carer Action Plans must be attached to the Carer Record within Carers ACT CRM
system and procedural guidelines
Teamwork • Work individually and collaboratively in the team to maintain a high quality,
professional service
• Attend staff, team and other meetings as required
• Support Volunteers who assist in the program (if any)
• Assist in cross-team or cross organisation activity
• Participate in and assist with the general operations of the organisation
Professional • Attend relevant group seminars and training sessions
Development • Complete training as directed by the organisation including online training through
our learning management system (Sentrient)
• Develop and implement individual program for professional development
Policy and Procedures, • Comply with Carers ACT’s policies, procedures and work instructions
compliance and risk • Apply Carers ACT’s risk management framework to manage risks associated with
management service delivery
• Value diversity and participate in supporting a respectful and inclusive workplace
culture

Selection Criteria
Essential: • Qualifications in community services, health, aged care, mental health or disability
fields
• Experience and/or qualifications in the delivery of care planning, case management
or intake and assessment processes
• Demonstrated empathetic and flexible approach to manage the sensitivities of carer
issues
• Excellent computer skills including proficiency in Microsoft Office suite and use of
databases
• Highly developed client documentation, record keeping and file management skills
with an attention to detail
• Ability to build and nurture relationships and networks with service providers and
target groups and within the ACT community
• Highly developed organisational, communication and interpersonal skills (both as part
of a team as well as individually) and competency in liaising with a wide range of
stakeholders
• Demonstrated understanding of caring issues within a community environment to
coordinate quality service delivery
• Demonstrated understanding of privacy legislation and ability to work within these
parameters
• Ability to assess and evaluate changes in individual behaviour necessary to
respond to planned interventions.
• Current Working with Vulnerable People Registration
• Current National Police Record Check
Current Driver’s License (essential to undertake client meetings at locations other
than the Holt office)
Note: The functions of this role may change to meet future business needs or changes in current
contracts.
