The Court finds that notice has been given as required by law. (Welfare and Institutions Code, § 293.)

The Court has read and considered the social worker’s report(s) prepared for today’s hearing, and the testimony
and arguments, if applicable, and makes the following findings and orders:

The Court finds that continued jurisdiction is necessary because conditions continue to exist that justified the
Court in taking jurisdiction under Welfare and Institutions Code section 300.

The Court finds by clear and convincing evidence that return of the child to the physical custody of the
parent(s)/legal guardian(s) would create a substantial risk of detriment to the child, creating a continued
necessity for and appropriateness of the current placement.

The parent(s)/legal guardian(s) were ordered to participate in services as set forth in the case plan. The Court
finds that the extent of progress made toward alleviating or mitigating the causes necessitating placement:

By parents have not been substantial.

The Court finds that the Department of Children and Family Services has complied with the case plan by
offering or providing reasonable and/or active effort to enable the child's safe return home and to complete and
finalize the permanent placement of the child.

Reunification services are terminated for the parents.

The Court orders the matter set for a 26 Permanency Planning Hearing on Tuesday, August 24, 2021.

Minor remains a dependent of the Court pursuant to Welfare and Institutions Code §300. Order of Suitable
Placement remains in full force and effect.

Department of Children and Family Services is ordered to:

- Provide Permanent Placement Services to the minor.


- Obtain the minor's birth certificate.

Department of Children and Family Services is ordered to prepare a/an 26 Report by Tuesday, August 10, 2021.

The Department of Children and Family Services is directed to prepare an assessment plan as required by WIC
366.21(i).

The Department of Children and Family Services is ordered to initiate an adoptive home study within the next
week.

The Department of Children and Family Services shall request the child's birth records and birth certificate if
not previously provided.

CONFIDENTIALITY ADMONISHMENT IS GIVEN.

12/25/2020; (3) LMI filed 1/13/2021; (4) Supplemental Report filed 2/25/2021; (5) Notice of Hearing filed

DCFS to assist the paternal grandmother/caretaker in obtaining passports for all minors.

Paternal Grandmother is designated the co-educational rights holder for all minors and Minors’ counsel is to
submit the forms for signature, over the objection of the parents.
Parents are found to be in minimal compliance.
Father is present and ordered to return.
County Jail removal order is ordered for Mother for 08/24/21.
DCFS to facilitate visits for Maternal Grandmother in Nevada in summer and when the minors are not in
school.

Parents are ordered to appear electronically on the next Court date unless they receive notice that in-person
appearances are to resume and then they are to appear in person for the next date.

Minor is not to be removed from the Eight Southern California Counties without DCFS permission and is not to
be removed from the United States without a Court Order.

ALL REPORTS ARE TO BE SUBMITTED IN THE COUNTY WIDE PORTAL BY 8:30 AM ON THE
RECEIPT OF REPORT DATE. IF THE REPORT IS NOT SUBMITTED ON THE CORRECT DATE AND
TIME, THE SOCIAL WORKER IS ORDERED TO BE PRESENT ON THE RECEIPT OF REPORT DATE
BY 9:00 AM. SANCTIONS MAY BE IMPOSED IF THE REPORT IS NOT SUBMITTED TIMELY.

Court orders:

- No corporal punishment.
- No visitation by anyone under the influence of drugs or alcohol.

Department of Children and Family Services is ordered to prepare a/an RPP Report by Wednesday, October 13,
2021.

All prior orders not in conflict shall remain in full force and effect.

prepare a report, DCFS is to notice all appropriate parties.
