    b. All KE Medical cards (self & Family), Allianz card (if any) and RFID Card
    c. Copy of Last Paid Residential KE Bill
    d. Zakat Declaration CZ-50 form along with two witnesses’ signature mentioning their CNIC (Dated: Before March 12, 2021 or
    before that for current year)
    e. Last Updated Attendance sheet from Biometric system signed by HOD

    f. Copy of (Employee) Death Certificate issued by NADRA.
    g. Succession certificate issued by court. (Heirs can submit later)
