I am the mother of . She has been found to have a solid mass in her breast area and as per consultation
by the doctor, it is recommended to be extracted as soon as possible. With that, I am writing this letter
to ask permission to excuse my daughter from attending your course orientation tomorrow, August 18,
2021, as her operation is scheduled on the said date at Cebu Doctor’s Hospital. Together with this letter
is the doctor’s note about the medical procedure she will undergo. As discussed by the doctor, she will
be sent home after her operation, thus, it will not affect her future online meeting attendance with your
subject.

permission if I could excuse myself for tomorrow's course orientation with the said
subject. I attached herewith the doctor's prescription about the medical procedure I will
undergo and an excuse letter signed by my mother as per instruction to your course
policy. I apologize if I could not attend tomorrow's meeting. I commit to read the syllabus
you provided.
