Form Instructions
1. Put applicable information into green boxes as follows.
2. Fill out Current Fuel Index (CFI) and Base Fuel Index (BFI) information near the top of the form
3. Fill in quantities of Material Moved for any items used.
4. Look at the Fuel Cost Adjustment value (FCA) in the yellow box at the top of the form for final value.
If the value is zero, there is no adjustment because the fuel calculation does not exceed the
less than 0.85 or more than 1.15 criteria for adjustment by specification.
