1. GENERAL CONTEXT AND SITUATION


What is the context of my screening assessment?

 This assessment screening aims to understand deeper those children with


challenges and provide them the proper intervention they are needed.

2. Stakeholders
A. Who are my 3 most important stakeholders? Who will benefit the screening?

3. REASONS AND PURPOSE OF ASSESSMENT


What is the typical reasons for assessment?/ What are the primary purpose of the
assessment?

1. to help the children to overcome their challenges

2. to provide appropriate intervention for the child's welfare

3. to let the other family members to be aware and give psychological support to
children and not be in denial of their child's welfare

 fell many times on stairs


 almost drown in the sea many times
 has uncle that suffering from mental health

General Observation:

 As I observed the child, I noticed that her behavior is different from other children. She
seems to be fine and energetic but most of the time, whenever you talk to her she act like
there's something wrong on her behavior. She even imitate other children's actions when
she's angry. Another is, she always act like crazy people's actions and always not on the
mode. And when she is asked to buy something in the store, she can't remember the one that
you'd asked her to buy, she comes back to house and asked you again on what will she buy.
But when it comes to incentive money, she will never forget it, especially if you have told her
that you will give her 10 pesos, she will surely remember it even already a week ago. Hence,
this child experienced several accidents before and it is really traumatic for a child like her.
Like she fell out on the stairs and got injured. She was also nearly drown in the sea when
she was 5 years old and in when she was baby she fell on the bed. And I also found out that
her uncle that gave her name is suffering from mental health.

Screening Assessment:
 Conducted one on one interview with the child and asked her about the experiences that
she had. She then shared and narrated everything about her experiences.
 Conducted an interviewed to the mother and assure that what the child had shares are truth
 Behavioral Observation where there's a need to observe the child's behavior and see what
are the things that need to be improved
 Parent's psychological support to the child is very needed and much important
 Always cater what the child needs and interest

Recommendations
 As for me, the child should always have someone to be with her and care for her, because I
believe that she had suffered very much trauma due to the accidents she had experienced.
Psychological support to the child is much needed. And don't let the child feel isolated and
punished her for not being able to do what you'd asked her to do because it causes the child
to think and do such undesirable actions.
