AFFIDAVIT OF AGREEMENT

(APIDABIT SA KASUGTANAN)

            I, Juan Dela Cruz, Filipino citizen, of legal age, married and a resident of Brgy. Sta. Rita,
Maasin, Iloilo after having been duly sworn in accordance with law, hereby depose and say:
           
(Ako si,  Juan Dela Cruz, Filipino, may legal nga pang edaron, may asawa kag residente sang
Brgy. Sta. Rita, Maasin, Iloilo pagkatapos manumpaon suno sa layi, nagasaysay sang masunod:)

1. That on August 16, 2021 I was re-appointed as Reliever Guard at RNG-Iloilo.

(Sang Agosto 16, 2021 nare-appoint ako silbi isa ka Reliever Guard sang RNG-Iloilo)

2. That as Reliever Guard, I agree to only about 5-6 duties in a 1-15 day shifting duty
schedules.
(Apang bilang isa ka Reliever Guard, nagasugot ko sa 5-6 nga duty sa sakop sang 1-15
shifting duty skedjul.)

3. That I fully understood that this is only temporary until such time that an additional post
will be available for regular duties.
(Nakaintiende ako nga ini temporary lang mentras tanto wala pa may dugang nga post
para sa regular nga duty.)

4. That I fully understand the exigency of the situation to fully comply with the contractual
obligations of the agency to the client.
(Naintiendihan ko nga ini gibuhat tion sa pagakinhanglanon sang panahon kag masunod
ang mga obligasyon suno sa kontrata sang ahensya kag sang kliyente)

5. That I'm amenable with this set up and as specifically stated under item no. 2 of this
affidavit of agreement
(Nagasugot ako sa ini nga set-up suno sa ginasiling sa ika 2 nga salaysay sang sini nga
kasugtanan.)

This Affidavit of Agreement was made this August 16, 2021. I have hereto attached my
signature to aver to the veracity of its contents and concurred by our Witnesses.
