1. What is nutrition?
 Is a biochemical and physiological process by which an organism uses food to support
its life. It includes ingestion, absorption, assimilation, biosynthesis, catabolism, and
excretion.

2. What are the common problems of nutrition?


 Maternal nutritional anemia
 Protein energy malnutrition
 Vitamin A deficiency
 Lactation failure
 Addiction to milk feeding; and
 Inadequate preparation and use of artificial milk products

3. Give at least 5 nursing intervention for the client with nausea and vomiting.
 Provide an emesis basin within easy reach of the patient.
 Educate and assist patient about oral hygiene.
 Assist the patient in diagnostic testing preparation.
 Eliminate strong odors from surrounding.
 Maintain fluid balance in patient at risk.

4. What is NGT?
 Nasogastric Tube - a tube that is passed through the nose and down through
nasopharynx and esophagus into stomach.

5. Enumerate the step by step procedure in inserting NGT.


 Perform hand hygiene, peppered supplies at bedside.
 Stood on same side of bed as naris chosen for insertion, physician-patient
appropriately, obtain assistance if necessary.
 Applied pulse oximeter, measure vital signs, ensure patient stability before inserting
tube.
 Please bath towel over patient’s chest, keep facial tissues within reach.
 Determine length of tube to be inserted, mark location properly.
 Prepare NG or nasoenteric tube for intubation.
 Obtain order for stylet tube.
 Inject water from syringe into the tube if tube has guidewire or stylet.
 Ensure stylet is positioned securely within tube, inject water from syringe into tube.
 Cut hypoallergenic tip or prepare other securing device.
 Apply clean gloves.
 Dip tube with surface lubricant into room temperature water or applied lubricant.
 Handed alert patient a cup of water with straw.
 Explain next step, insert tube through nostril to back of throat, aimed properly.
 Had patient flex head to work chest at appropriate time.
 Encourage patient to swallow with small sips of water, advance tube as patient
swallow, rotate tube while inserting.
 Reemphasize mouth breathing and swallowing.
 Did not advance tube during inspiration or coughing, monitor oximetry and
capnography
 Advance tube each time patient swallow, until desired length had been passed.
 Check for position of tube in back of throat.
 Anchored tube nose temporarily.
 Check placement of tube by aspirating stomach contents.
 Anchor tube the patient’s nose, mark exit site on tube, select appropriate option for
anchoring.
 Apply membrane dressing or tube fixation device properly.
 Apply tape properly.
 Fastened end of NG Tube properly to patients gown.
 Assist patient to a comfortable position.
 Remove gloves, perform hand hygiene.
 Obtained x-ray film or chest/abdomen
 Perform hand hygiene, apply clean gloves, administer oral hygiene, cleans tube at
nostril properly.
 Remove gloves, dispose of equipment, perform hand hygiene.
 Remove NG Tube:
 1) verified order for tube removal
 2) gather equipment
 3) explain procedure to patient
 4) perform hand hygiene, applied gloves.
 5) position patient appropriately
 6) place disposable pod over patient’s chest
 7) disconnected from feeding administration set
 8) remove securement device from the patient’s nose
 9) instruct patient to take a deep breath and hold, kinked end of tubing, withdrew
tube in one motion, dispose of tube appropriately.
 10) offer tissue to patient
 11) offer mouth care
 12) remove gloves and perform hand hygiene

6. Define and explain elimination.


 Urinary elimination is the last step in the removal and elimination of excess water and
byproducts of body metabolism.

7. Enumerate the step by step procedure of inserting IFC.


 Check patients plan of care for size and type of catheter, use smallest size possible.
 Perform hand hygiene.
 Provide privacy.
 Raised bed to appropriate height, raise side rail on opposite side, lower side whale on
working.
