Paradigm to create Sense of Safety in Personal. With use Personal Protective Equipment
(PPE) when do the job in field and always implemented of Standard Operating Procedure
(SOP),

• Monitoring piping crew to conduct join pipe with all of the process. Preparation the personnel
, document , material and etc,..Safety Meeting to share the Hazard on Job in Job Safety
Analysis and Standard Operating Procedure and Execution from the brushing the pipe,
create the design of pipe cutting , grinding and weld the pipe. until the end of job.

• Remind Pipe fitter, Welder , foreman , helper and fire watcher always use PPE on job Safety
Hat , Safety Shoes, Safety Glasses , Uniform, Face shield , Apron , Hand Glove and etc
accord their position.

• Monitoring Crane crew to conduct Lifting activities .Preparation the personnel , document ,
material and etc,..Safety Meeting to share the Hazard on Job in Job Safety Analysis and
Standard Operating Procedure and Execution from the Crane Preparation by PTI, Operate
the Out of Rigger process with SOP , move the boom of crane with signal man code , down
the boom ,then install the sling , Rigger conduct tie in the object (pipe, etc) shall be adjust
with the Safe Working Load (SWL) of Crane and sling. Up the sling , move the boom of crane
left and right and STOP on the exact job position, and down with safely. All crane moving are
shall be clear from peoples , Especially job area near from Power line , building and main
road etc. Make sure to keep safe distance and barricade always safe position.

• Monitoring Earth work activities . Preparation the personnel , document , material and
etc,..Safety Meeting to share the Hazard on Job in Job Safety Analysis and Standard
Operating Procedure and Execution from the Equipment (Dozer , Excavator etc)

activities : provide a borrow pit area , Well pad area and etc. Usually this activities start from
the new area like jungle. Dozer to cut trees and the hill of land to clear area based on
Construction Execution Plan (CEP) the client. TheHazard in this activity are bees on the trees
or in the land are very danger and the land position etc. The operator of equipment must be
make sure area are save and use Bee net when operate in cabin. Close the equipment cabin.

• Monitoring Civil activities . Preparation the personnel , document , material and etc,..Safety
Meeting to share the Hazard on Job in Job Safety Analysis and Standard Operating
Procedure and Execution base on Safety in Design and Construction Execution Plan (CEP).
Civil activities generally use the Chemicals and this high Hazard in this activity, to minimize
the risks, HSE team must be remind all team to complete the Material Safety Data Sheet and
USE specific PPE for Chemicals Hazard. For Working at height, the crew shall be present in
Specific Task Training (i.e.. Working At Height , Confined Space etc) it is important to
understand the Hazards and to complete the Permit. HSE team, control the use of Full Body
Harness , install of Scaffolding (minimum safe distance from electrical conductors for the
erection of scaffolding i.e. Not Exceeding 66 Kv minimum distance 4.0 m) , Barricade etc. All
of that must be clear and safe to use.

• Monitoring Electric activities . Preparation the personnel , document , material and


etc,..Safety Meeting to share the Hazard on Job in Job Safety Analysis and Standard
Operating Procedure and Execution base on Safety in Design and Construction Execution
Plan (CEP). Electric activities generally Working at Height with high risk from electrically. to
minimize the risks, HSE team must be remind all team to complete the Material Safety Data
Sheet and USE specific PPE for Electrical Hazard. For Working at height, the crew shall be
present in Specific Task Training (i.e.. Working At Height , Confined Space etc) it is important
to understand the Hazards and to complete the Permit. HSE team, control the use of Full
Body Harness , Barricade etc. All of that must be clear and safe to use.

• Monitoring Survey activities . Preparation the personnel , document , material and etc,..Safety
Meeting to share the Hazard on Job in Job Safety Analysis and Standard Operating
Procedure base on Safety in Design and Construction Execution Plan (CEP). As the first
team which open the new are to provide the coordinate base on the Project Execution Plan
(PEP), the crew shall be use with Bee net, Safety Shoes , provide Snake bit , First Aid etc.
