state Nigeria, school children were clinically examined for evidence of malnutrition and oral health status in a UNICEF supported programme. It was found that 80% of the primary school children were underweight , 20% had angular stomatitis and 40% were stunted. A routine or regular health information system is the collection of data which usually consists of a mechanism ( registration procedure) for collecting the data as they become available e.g data on births, marriages, diseases e.t.c while ad- hoc is a collection of data which is usually in form of a survey of collected data that is not available on regular basis, been that the data collected maybe for research or administrative purpose. 5b.
