History
The first known case of surgical removal of the appendix occurred in 1735 by Claudius
Amyand in London. It was not until 1886, however, after Reginald H Fitz read his paper to
the first meeting of the Association of American Physicians in Washington DC, that the early
surgical removal of the appendix became the standard treatment for appendiceal
inflammation. For over 100 years, the technique of open appendicectomy remained the gold
standard. It allowed a safe, relatively simple way for removal of the appendix and it has been
a procedure that new surgical trainees used to acquire basic surgical skills. In the 1990s, a
laparoscopic approach to the appendix was popularized and is now often the preferred
approach allowing some diagnostic and therapeutic benefits. A recent Cochrane review cited
laparoscopic advantages to be decreased postoperative pain, decreased hospital stay, earlier
return to work, and less wound infections, although at the expense of increased intra-
abdominal abscesses. However, open appendicectomy remains a valid option especially in
centers where laparoscopic expertise is unavailable and where the benefits would be minimal,
such as in thin males where there is no diagnostic uncertainty and also possibly in children.
Masked Diagnosis
Retrocecal appendicitis may be more severe than apparent on palpation due to the
dilated cecum acting as a buffer between the inflamed appendix and the anterior abdominal
wall.
Appendicitis in a long retrocecal appendix or with a cecum situated high in the right
hypochondrium may mimic acute cholecystitis. A long intrapelvic appendix may present as a
pelvic abscess and be confused with tubo- ovarian pathology in women.
Particular care must be taken in elderly and immunocompromised patients, in whom the
clinical signs of appendicitis may be masked, as may the diagnosis.
Special Situations
An appendiceal mass should usually be treated conservatively with antibiotics and
bowel rest. Subsequent investigation with colonoscopy and CT scan is appropriate and
possible interval appendicectomy carried out.
Appendicitis during pregnancy is managed best by early appendicectomy. Miscarriage
and early onset labor are unusual with surgery which is safest in the second trimester.
Consent
Consent should be obtained with discussion of the general risks of surgery and
anesthesia with specific mention of postoperative wound infection, intra-abdominal abscesses
and the possibility of finding alternative pathology requiring extension of the incision or
making a midline incision.
Anesthesia
A fasting time of 6 hours for solids and 2 hours for fluids is preferred. General
anesthesia is used with endotracheal intubation and muscle relaxation.
OPERATION
Position Of The Patient
The patient is placed in the supine position. Calf compressors are used, if available, in
adults. An adhesive diathermy pad is applied to the patient’s thigh.
Incision
The abdomen should be palpated before anesthesia to determine the point of maximal
tenderness. The abdomen is palpated again following anesthesia and muscle relaxation to
facilitate palpation of a mass. These factors, as well as concern for cosmesis, will determine
the area over which the incision should be placed. The classic incision is the grid iron. This
involves an oblique skin incision in the line of fibers of external oblique centered over
McBurney’s point. This is the point at the junction of the lateral third and medial two-thirds
of a line passing from the anterosuperior iliac spine to the umbilicus, the classic surface
marking for the base of the appendix. The preferred incision may be a transverse incision in
the skin crease line starting approximately 2 cm medial to the iliac crest. A midline incision is
appropriate for generalized peritonitis or when a large mass is palpated. If laparoscopy has
been performed initially and a decision then made to convert to open surgery, the optimal
position for the incision can be determined by palpating the abdominal wall over the position
of the appendix, while observing the laparoscopic monitor (Figure 5.1.1).
Preparation Of Abdomen
The entire abdomen is prepared with a Betadine or chlorhexidine solution to allow
extension or change of incision if required. The right lower quadrant is square draped to also
allow access to the midline and the right flank above the anterior iliac crest.
Wound Closure
Free turbid fluid should be completely suctioned from the peritoneal cavity. If frankly
purulent fluid is found, then warm normal saline should be poured into the right iliac fossa
and aspirated until the return is clear. If no contamination of the peritoneal cavity is observed,
no washout is necessary. An intraperitoneal drain is unnecessary in most cases. Wound
retractors are placed deep to the muscles and outside the peritoneum. Small artery forceps are
placed at either end of the peritoneal incision and in the middle of its upper and lower edges.
A continuous 2-0 Vicryl suture is inserted, ensuring at all times that intestine is not
inadvertently caught by the needle (Figure 5.1.11).
Further saline lavage is applied to the wound before muscle closure. If a muscle
splitting incision has been done, the muscles of internal oblique and transversus abdominis
are now approximated with interrupted 2-0 Vicryl sutures. The fascia of the internal oblique
muscle rather than the muscle bundle should be picked up with minimal tension, thus
avoiding ischemia (Figure 5.1.12).
A continuous 1 Vicryl or PDS is used to close the external oblique muscle, preferably
burying the knot in thin patients. Scarpa’s fascia can be approximated with interrupted
absorbable sutures and the skin closed with a 3-0 Monocryl subcuticular suture. If significant
pus has been encountered, interrupted sutures are more appropriate and it may even be
prudent to leave the wound partially or completely open or, alternatively, a small corrugated
drain may be left in the subcutaneous space (Figure 5.1.13).
SPECIAL CIRCUMSTANCES
Medial Wound Extension
By incising the lateral edge of the rectus sheath, if not done so already, the rectus
muscle is exposed allowing further medial retraction. Rectus muscle can be divided
transversely with diathermy to further increase exposure. Care must be taken to avoid
damaging the inferior epigastric vessels, which pass posterior to the rectus muscle (Figure
5.1.14).
Lateral Woundextension
This is necssary to deal with a high retrocecal or subhepatic appendix. The skin incision
should be extended above the iliac crest towards the lateral abdomen and flank. The muscle
layers may be further split laterally, but fibers of the internal oblique and transversus
abdominis muscles may need to be divided to gain optimum exposure. The use of a narrow
retractor and laparoscopic instruments can also assist in this situation. An atraumatic
laparoscopic bowel grasper can hold and retract a high cecum to aid division of adhesions
and further mobilization can be achieved by use of an alternative energy source, such as a
harmonic scalpel (Figure 5.1.15).
Retrograde Appendicectomy
Having exposed the base of the appendix at its junction with the cecum, an artery
forceps is passed through the mesentery and the appendix base is clamped, ligated, and
divided as described above. The mesoappendix is then secured from the base of the appendix
towards its tip by serial clamping, division, and ligation. At each stage, the cecum is gently
pushed superiorly within the abdominal cavity to give exposure to the portion of
mesoappendix being ligated (Figure 5.1.16).
When it is necessary to extend the incision laterally, incising the lateral peritoneum in
the right paracolic gutter will allow the cecum and ascending colon to be gently pushed
medially, thus exposing more of the elongated appendix (Figure 5.1.17).
Appendiceal Tumor
If the appendix contains a tumor less than 2 cm in diameter, confined to the appendix
and involving neither the mesoappendix nor the base of the appendix, appendicectomy should
include as much mesoappendix as possible. Most of these will be carcinoid tumors. Care
must be taken to remove a mucin-filled appendix intact. Patients who have an obvious
appendiceal adenocarcinoma, any neoplasm greater than 2 cm, or involvement of the base or
mesoappendix should be considered for immediate right hemicolectomy to achieve optimal
outcome (Figure 5.1.18).
COMPLICATIONS
Wound infection
The wound dressing may be removed after 24 hours to allow frequent inspection. Minor
cellulitis may be treated with appropriate antibiotics. Wound collections are easily treated by
removing a suture and probing to release accumulated pus.
Intra-Abdominal Collection
Increasing abdominal or pelvic pain with diarrhea and fever heralds a peritoneal abscess. An
abdominal CT scan
should be performed with radiological drainage carried out and appropriate antibiotics
commenced if a collection is found.
Intestinal Fistula
This is a rare complication. Conservative treatment is appropriate initially, if there are no
signs of peritonitis.
History
Kasus operasi pengangkatan usus buntu pertama yang diketahui terjadi pada tahun 1735

penurunan rawat inap di rumah sakit, kembali bekerja lebih awal, dan infeksi luka yang lebih
sedikit, meskipun dengan mengorbankan peningkatan abses intra-abdomen. Namun, usus
buntu terbuka tetap menjadi pilihan yang valid terutama di pusat-pusat di mana keahlian
laparoskopi tidak tersedia dan di mana manfaatnya akan minimal, seperti pada pria kurus di
mana tidak ada ketidakpastian diagnostik dan juga mungkin pada anak-anak.
Diagnosis Bertopeng
Apendisitis retrosekal mungkin lebih parah daripada yang terlihat pada palpasi karena
sekum yang melebar bertindak sebagai penyangga antara apendiks yang meradang dan
dinding perut anterior.
Apendisitis pada apendiks retrosekal yang panjang atau dengan sekum yang terletak
tinggi di hipokondrium kanan dapat menyerupai kolesistitis akut. Apendiks intrapelvis yang
panjang dapat muncul sebagai abses pelvis dan dibingungkan dengan patologi tubo-ovarial
pada wanita.
Perhatian khusus harus diberikan pada pasien lanjut usia dan pasien
immunocompromised, di mana tanda-tanda klinis apendisitis dapat ditutupi, seperti halnya
diagnosis.
Situasi Khusus
Massa apendiks biasanya harus dirawat secara konservatif dengan antibiotik dan
istirahat usus. Investigasi selanjutnya dengan kolonoskopi dan CT scan adalah tepat dan
kemungkinan dilakukan apendisektomi interval.
Apendisitis selama kehamilan paling baik ditangani dengan apendisektomi dini.
Keguguran dan persalinan dini tidak biasa dengan operasi yang paling aman pada trimester
kedua.
Izin
Persetujuan harus diperoleh dengan diskusi tentang risiko umum pembedahan dan
anestesi dengan menyebutkan secara spesifik infeksi luka pasca operasi, abses intra-abdomen
dan kemungkinan menemukan patologi alternatif yang memerlukan perluasan insisi atau
membuat insisi garis tengah.
PENILAIAN DAN PERSIAPAN PREOPERATIF
Cairan intravena harus diberikan dan pasien direhidrasi secara memadai terutama jika
muntah telah menjadi ciri yang menonjol. Dosis tunggal antibiotik spektrum luas sebelum
operasi diberikan untuk menutupi organisme aerobik dan anaerobik dan harus dilanjutkan
jika ditemukan peritonitis atau perforasi yang signifikan. Seluruh perut dicukur untuk
memungkinkan sayatan garis tengah dalam kasus di mana diperlukan. Pencegahan deep vein
thrombosis sangat penting. Tindakan, termasuk mobilisasi dini dan stoking antiemboli, wajib
dilakukan dengan penggunaan natrium heparin subkutan atau heparin berat molekul rendah
yang digunakan pada pasien yang sesuai. Kateter urin menetap dan selang nasogastrik tidak
digunakan secara rutin, tetapi harus digunakan pada pasien dengan peritonitis umum.
Posisi Pasien
Pasien ditempatkan dalam posisi terlentang. Kompresor betis digunakan, jika tersedia,
pada orang dewasa. Bantalan diatermi perekat diterapkan pada paha pasien.
Anestesi
Waktu puasa 6 jam untuk makanan padat dan 2 jam untuk cairan lebih disukai. Anestesi
umum digunakan dengan intubasi endotrakeal dan relaksasi otot.
OPERASI
Posisi Pasien
Pasien ditempatkan dalam posisi terlentang. Kompresor betis digunakan, jika tersedia,
pada orang dewasa. Bantalan diatermi perekat diterapkan pada paha pasien.
Irisan
Abdomen harus dipalpasi sebelum anestesi untuk menentukan titik nyeri tekan
maksimal. Perut dipalpasi lagi setelah anestesi dan relaksasi otot untuk memfasilitasi palpasi
massa. Faktor-faktor ini, serta kepedulian terhadap kosmetik, akan menentukan area di mana
sayatan harus ditempatkan. Sayatan klasik adalah besi kisi. Ini melibatkan sayatan kulit
miring di garis serat miring eksternal yang berpusat di atas titik McBurney. Ini adalah titik di
persimpangan sepertiga lateral dan dua pertiga medial dari garis yang melewati spina iliaka
anterosuperior ke umbilikus, permukaan klasik yang menandai dasar apendiks. Insisi yang
lebih disukai dapat berupa insisi transversal pada garis lipatan kulit mulai kira-kira 2 cm
medial ke krista iliaka. Insisi garis tengah sesuai untuk peritonitis umum atau ketika massa
besar dipalpasi. Jika laparoskopi telah dilakukan pada awalnya dan keputusan kemudian
dibuat untuk mengkonversi ke operasi terbuka, posisi yang optimal untuk sayatan dapat
ditentukan dengan meraba dinding perut di atas posisi apendiks, sambil mengamati monitor
laparoskopi (Gambar 5.1.1).
Persiapan Perut
Seluruh perut disiapkan dengan larutan Betadine atau klorheksidin untuk
memungkinkan perluasan atau perubahan sayatan jika diperlukan. Kuadran kanan bawah
berbentuk bujur sangkar untuk juga memungkinkan akses ke garis tengah dan sayap kanan di
atas krista iliaka anterior.
Divisi Mesoappendix
Arteri apendiks muncul dari cabang posterior arteri ileokolika. Cabang-cabang arteri
appendix kemudian berjalan melalui mesoappendiks menuju appendiks. Untuk mendapatkan
paparan yang memadai, lipatan peritoneum di bawah sekum mungkin memerlukan
pembelahan untuk memberikan mobilisasi yang memadai. Apendiks diperlihatkan di antara
sepasang tang Babcock di ujung dan yang lain di dekat pangkal. Jendela kecil dikembangkan
di persimpangan mesenterium dan usus buntu menggunakan diatermi atau forsep arteri.
Pedikel yang dihasilkan yang mengandung cabang-cabang pembuluh darah apendiks diligasi
dan dibagi. Jika mesenterium tebal, edematous, atau pendek, jahitan mungkin diperlukan
(Gambar 5.1.7).
KEADAAN KHUSUS
Ekstensi Luka Medial
Dengan menorehkan tepi lateral selubung rektus, jika belum dilakukan, otot rektus
terbuka sehingga memungkinkan retraksi medial lebih lanjut. Otot rektus dapat dibagi
melintang dengan diatermi untuk lebih meningkatkan paparan. Perawatan harus dilakukan
untuk menghindari kerusakan pembuluh darah epigastrium inferior, yang melewati posterior
otot rektus (Gambar 5.1.14).
Apendiktomi Retrograde
Setelah mengekspos dasar apendiks di persimpangannya dengan sekum, forsep arteri
dilewatkan melalui mesenterium dan dasar apendiks diklem, diikat, dan dibagi seperti
dijelaskan di atas. Mesoappendiks kemudian diamankan dari dasar apendiks ke ujungnya
dengan penjepitan serial, pembagian, dan ligasi. Pada setiap tahap, sekum didorong dengan
lembut ke superior di dalam rongga perut untuk memperlihatkan bagian mesoappendiks yang
diligasi (Gambar 5.1.16).
Jika perlu untuk memperluas insisi ke lateral, insisi peritoneum lateral di parokolik
talang kanan akan memungkinkan sekum dan kolon asendens didorong dengan lembut ke
medial, sehingga memperlihatkan lebih banyak apendiks yang memanjang (Gambar 5.1.17).
Tumor Apendiks
Jika apendiks berisi tumor dengan diameter kurang dari 2 cm, terbatas pada apendiks
dan tidak melibatkan mesoappendiks maupun dasar apendiks, apendiktomi harus mencakup
sebanyak mungkin mesoappendiks. Sebagian besar akan menjadi tumor karsinoid. Perawatan
harus diambil untuk menghapus apendiks yang berisi musin utuh. Pasien yang memiliki
adenokarsinoma apendiks yang jelas, setiap neoplasma yang lebih besar dari 2 cm, atau
keterlibatan basis atau mesoappendiks harus dipertimbangkan untuk dilakukan
hemikolektomi kanan segera untuk mencapai hasil yang optimal (Gambar 5.1.18).
KOMPLIKASI
Infeksi luka
Pembalut luka dapat dilepas setelah 24 jam untuk memungkinkan pemeriksaan yang
sering. Selulitis minor dapat diobati dengan antibiotik yang sesuai. Kumpulan luka mudah
dirawat dengan melepas jahitan dan probing untuk melepaskan akumulasi nanah.
Koleksi Intra-Perut
Meningkatnya nyeri perut atau panggul dengan diare dan demam menandakan abses
peritoneal. CT scan perut
harus dilakukan dengan drainase radiologi dilakukan dan antibiotik yang tepat dimulai
jika koleksi ditemukan.
Fistula Usus
Ini adalah komplikasi yang jarang terjadi. Pengobatan konservatif pada awalnya tepat,
jika tidak ada tanda-tanda peritonitis.
