YOUR PERMISSION FOR SYKES TO DISCLOSE YOUR PERSONAL INFORMATION

TO OUR CLIENT FOR BUSINESS PURPOSES

We are thrilled to have you join the SYKES team. Our company is a market leader in serving the customers of top global
companies. Our success is a result of hiring exceptionally ethical and caring people, like you, who are an outstanding fit to
serve those who call us for help. Your job is very important and you will have access to our clients’ and their customers’
highly confidential and personal information.

Part of SYKES’ company culture is complete transparency. When we set up working agreements with our clients, we build
in regular audits so our clients can frequently see the excellent work provided by our teams. These audits mean we share
information about you with our clients for business purposes, related to the work you will be performing. Additionally,
your personal information may be provided to the client to create your logins for their systems.

Your signature below indicates you agree and authorize Sykes Enterprises, Incorporated to release to the client, for the
account to which you are assigned, the following documents and information:

• Name, Social Security Number, driver’s license number, date of birth, home address, email address, and phone
number
• Drug screen results, which includes, but it not limited to a screen that tests for cocaine, amphetamines,
methamphetamines, marijuana, and opiates
• Confirmation of a completed background check, including criminal records, results from a national sex offender
database search, social security trace, education verification, E-Verify results
• Fingerprints
• I-9 Documentation, including photo identification
• Employment history, resume, and employment application documents, as well as personal references
• Training records, including, but not limited to, Data Security and HIPAA training
• Personnel files and employee records

Unfortunately, refusing to authorize SYKES to share such records with our clients will likely prevent us from scheduling
you to work at our company. You can request and receive a copy of this form after you sign it. You are also welcome to
revoke consent at any time. In that case, all that would be required is to provide your Human Resources representative
with a written notice that you no longer provide consent for SYKES to share your information. Revoking your consent will
also likely prevent us from scheduling you to work at SYKES. Your signature on this document indicates that you’ve read
this thoroughly, you’ve received clarification on any points you did not understand and that you are voluntarily signing
this in order to officially become a part of the SYKES People Serving People team. Unless you notify us otherwise, this
document will remain in effect throughout your employment with SYKES. Finally, your signature indicates that you agree
that a photocopy or facsimile of this consent form may be accepted with the same authority as the original.
