AFFIDAVIT OF LOSS

I, LOURDES C. FRONDA, of legal age, Filipino, and with residences at Block 21

Lot 37, Adelina 3 Phase 3 Extension, Brgy. Sto. Tomas, Biñan, Laguna, respectively, after
being sworn in accordance with law, depose and state:

1. I am the owner of a cellular phone with installed Sim Card No.

09063451990 which is a prepaid subscription from Globe Telecom.

2. On 31 October 2021, while we were traversing the highway along Acienda

Designers Outlet in Silang, Cavite, my cellphone, together with the said sim card
number, was snatched and lost. Despite diligent search, the same was lost, or otherwise
destroyed and could no longer be found.

3. Therefore, I am executing this Affidavit to attest to the truth of the

foregoing facts and for the purpose of my application for the re-issuance of the same
sim card number and for any legal intent and purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hand this 01 November 2021 in
