RE: Mrs. Margaret Goldstein, 61 years old

Dear Ma’am/Sir,

I am writing to transfer back to your care Mrs. Margaret Goldstein, she is admitted to our facility due to dislocated right
elbow. She is for discharged and for your continuous management is requested.

Mrs. Goldstein was admitted on 8th of November 2017 after she fell accidentally in the Homecare. She had injury as a
result cuff with sling applied, in addition slow recuperate noticed on her right elbow. She was commenced the
medication as follows Atacand plus 32/12.5 25mg per day, Rameron 15mg per day and lastly Oxydone 5mg three times a
day to be continued as needed.

During hospitalization Mrs. Goldstein developed urinary incontinence which require disposable adult diaper. Even
though she is hesitant to wear it her skin integrity is intact. She requires assistance in taking a bath and used ambulatory
assistive device such as wheelchair. She is medically stable now and plan to discharge on 12 th of November, 2017.

It is worth noting for to observe the recovery of Mrs. Goldstein right elbow as well her bladder incontinence care.
Moreover, scheduled follow up to physiotherapist has been done on 28 th of November, 2017 at 0200 pm in Eventide.
Kindly note Mr. Sol Goldstein will accompany her back to the Nursing Home.
