registered in the Department of Trade and Industry with Business Name No. 2579872
issued on January 28, 2021 with business address at Purok Morado, Pilit, Cabancalan,
Mandaue City;

2. That the said business is only a contact office and exist solely for the purpose of
receiving and processing orders;

3. That I am executing this affidavit to attest the veracity and truthfulness of the
foregoing statements and for whatever legal purpose this Affidavit may best serve.
