Guidelines For Awol: Reason of Awol Employee'S Action Requirements When Returning To Work Management Decision
An employee who intends to be absent from work should accomplish an Application for Leave Form. The application must be properly
accomplished and be recommended for approval by the immediate supervisor. A leave form is officially approved when signed by the Plant
Manager.
Accomplished leave form must be filed by the employee at least three (3) days before the intended date of leave. Submission of leave form is
not enough.
For instances that employee failed to accomplish leave form the following will apply:
The Admin In-charge will make a letter to the guards that these employees must present a RTWF to them before they will be allowed to
enter the plant’s premises
To acquire the Return To Work Form, the employee must coordinate with the supervisor. The RTWF thru text must only come from the
Company Admin CP.
Guidelines in the Issuance of RTWF
a. Employees on AWOL with considerable reason will automatically be issued with RTWF.
b. Employees on AWOL with unacceptable reason
