Failure of the Steel Wire Rope of Gripper System


GENERAL INFORMATION

Location: High Lift Pump House/Refinery

Nature of Incident: Near miss

Date of Incident: 18th May 2021

INCIDENT DESCRIPTION

At around 11:30 AM hrs., Contractor engineers were performing troubleshooting for the Gripper A of the
trash rack system of sea water intake facility system. The Contractor personnel gave command to the
gripper system to clean the Screen Pit chambers as part of the simulation. The gripper was stuck
between the bar screens when the hoist motor started to pull, which resulted in failure to one of the steel
wire ropes. No injuries to personnel and no damage to the gripper itself.

EARLY LEARNING POINTS

• Bapco OSOOL #2: Always work in a safe and controlled condition


• Bapco OSOOL #4: Always follow safe work practices and procedures and insist on other doing the
same
• Do it safely or not at all.
• Always keep eyes on the task, be vigilant and maintain awareness during activities.

IMMEDIATE ACTIONS

• Work stopped immediately and worksite secured for investigation.


• The incident communicated to concerned personal.

CORRECTIVE / PREVENTIVE ACTIONS

• IIR will be raised to investigate the incident to determine the root causes.
• Lesson learnt will be disseminated to all concerned personnel through safety meetings and safety
toolbox talks.
